About this Role 

This is a wonderful opportunity for a mission-driven team player to make a real difference in the lives of frail and vulnerable older adults in your community. In this role, you’ll bring warmth, kindness, and compassion to every interaction—whether you’re supporting participants with personal care, accompanying them to appointments, or sharing a smile during a group activity. You’ll work closely with families, caregivers, and our interdisciplinary care team (IDT) to ensure each participant feels safe, seen, and supported, while also staying attentive to any changes in their health or well-being.

What Does Success Look Like As Our PCA?
  • Offer Compassionate Daily Care – Help participants with everyday activities such as bathing, grooming, toileting, dressing, laundry, and meals—whether in the center, clinic, or occasionally in their homes.
  • Be a Friendly Companion and Guide – Assist participants as they get in and out of our transportation vehicles and accompany them on rides or to offsite medical appointments as needed.
  • Support Mealtimes with Heart – Make sure each participant receives the correct meal and offer a helping hand to those who need assistance with eating, always with patience and respect.
  • Help Keep Everyone Active and Comfortable – Provide gentle physical assistance throughout the day using proper techniques to ensure safety and avoid injury for everyone.
  • Pitch in with Food Service – Lend a hand with meal delivery and help create a welcoming and enjoyable dining experience.
  • Take Simple Health Measures – Monitor vital signs and keep a watchful eye on participant well-being as directed by the care team.
  • Keep Supplies Stocked and Ready – Ensure personal care supplies like incontinence items and grooming tools are available and organized.
  • Follow Team Guidance with Care – Carry out care tasks and status checks based on directions from our interdisciplinary team.
  • Connect with Warmth – Communicate clearly and kindly with participants, families, team members, and outside providers—building trust and teamwork every step of the way.
  • Jump In Where Needed – You’re a flexible team player who’s ready to lend a hand wherever it’s needed to ensure participants receive the best care possible.


  • What Does An Ideal Candidate Look Like?
  • A Heart for Service – You’re passionate about caring for older adults and feel called to support high-risk seniors and those with complex needs.
  • Hands-On Experience – You have experience as a Personal Care Attendant (PCA) or Certified Nursing Assistant (CNA), especially working with frail or elderly individuals.
  • Safety First – You’re CPR and First Aid certified—or willing to complete certification within 60 days of starting.
  • A Plus, Not a Must – CNA or Home Health Aide (HHA) certification is preferred but not required—we value your experience and heart just as much.
  • Ready to Go – You’re flexible and willing to provide care in participants’ homes when needed.
  • Bilingual Bonus – Speaking more than one language is a big plus in serving our diverse community.
  • Reliable Transportation – You have a valid California driver’s license, dependable transportation, a clean driving record, and current auto insurance—ensuring you can safely reach those who need you most.


  • What Else You Should Know?
  • Your Schedule – This is a full-time, Monday through Friday role with hours between 7:00 AM and 5:30 PM.
  • Onsite Connection – This position is 100% onsite, giving you the opportunity to build meaningful, in-person relationships with participants and your teammates.

  • What Are The Physical Requirements For This Role?
  • Ability to move independently and frequently throughout the workday, including walking, standing, bending, reaching, squatting, and kneeling for extended periods of time.
  • Ability to lift, push, pull, and carry participants and objects (such as equipment and supplies), up to a minimum of 50 pounds, and to assist with repositioning and transferring participants using proper body mechanics and assistive devices.
  • Ability to support and assist participants with mobility, including transfers (bed to chair, wheelchair to toilet, etc.) and safe ambulation with or without assistive devices.
  • Proficient use of hands and fingers to perform job-related activities such as feeding, bathing, dressing, grooming, and other hands-on personal care tasks.
  • Ability to perform repetitive motions and tasks throughout the workday in a fast-paced environment.
  • Sufficient hearing and vision to safely observe participants, respond to requests, read documentation, and hear alarms or safety cues.
  • Ability to use a laptop/computer, electronic medical record, and telephone independently to document care and communicate with the team.
  • Ability to work in a variety of environments, including a busy PACE day center, clinic, and participants’ homes, which may involve exposure to body fluids, odors, cleaning agents, and varying home conditions.

  • What Is The Working Environment For This Role?
  • This role works across multiple settings, including a busy PACE day center, clinic spaces, external appointments and participants’ homes within the community.
  • The day center and clinic environment include frequent interaction with participants, team members, medical equipment, and shared common areas.
  • Home visits may involve entering environments with varying levels of cleanliness, clutter, lighting, noise, and temperature, as well as potential exposure to pets, secondhand smoke, and different home layouts (including stairs).
  • The role may require riding in or driving a vehicle between sites and participant homes in all typical Southern California weather conditions.
  • Team members may be exposed to body fluids, odors, and cleaning agents and are expected to follow infection control practices and use personal protective equipment (PPE) as needed.
  • The environment is often fast-paced, with frequent participant contact, interruptions, and changing priorities throughout the day.
