Occupational Therapist - SmartLife chez Willow Valley Communities

Join the Willow Valley Communities SmartLife Team as a PRN (as-needed) Occupational Therapist! This Occupational Therapist will be responsible for completing home evaluations for SmartLife Members in the York, Lancaster and Lebanon areas on an as-needed basis.

Wage:

• $48/hour

Responsibilities:

• Models exemplary customer service and models care, compassion and trust toward Team Members and SmartLife Members.
• Completes timely clinical documentation to include functional goal-setting, program and therapeutic approach.
• Adheres to all applicable federal, state and local standards of care and ethics and policies concerned with the practice of Occupational Therapy.
• Attends meetings, seminars and participates in other activities to promote professional growth.
• Makes recommendations and consults with Lifecare Coordinator regarding methods for Members to maximize independence.
• Evaluates Members ability within the home setting to function independently.
• Evaluates the home environment of Members to promote independent living.

Qualifications:

• Bachelor's degree (B. A.) from an accredited occupational therapy program at a four year college or university
• Must be board certified and licensed by the state
• Computer literacy required
• Must have own motorized transportation to commute between Members homes as required
