Responsibilities

• Provide physical therapy services according to individualized care plans

• Evaluate and monitor patients’ progress and adjust treatments as necessary

• Communicate patient status and progress to the Physician, Case Manager, and Director of Nursing

• Maintain accurate and timely clinical documentation using company devices

• Supervise Physical Therapist Assistants (PTAs) as assigned

• Comply with all Health Link policies, procedures, and quality standards

Qualifications

• Current California Physical Therapist (PT) License – Required

• 1+ year of Home Health experience – Preferred

• Valid California Driver’s License and reliable transportation

• Must be able to cover Oakland and nearby communities

Compensation

$60.00–$70.00 per hour (commensurate with experience)
$5,000 Sign-On Bonus
