The PRN - Medical Master Social Worker is a qualified professional who provides medical social services to clients in their homes. These services are performed in accordance with physician orders and under the direction and supervision of the Hospice Medical Director or appropriate supervisor, and other duties as deemed necessary.
Qualifications
1. Provides services under the direction of a physician and in accordance with a plan of care.
2. Assists in the admission process of the patient to the hospice.
3. Assesses emotional factors related to terminal illness.
4. Assists the physician and other IDT/IDG members in recognizing and understanding the social/mental stress and/or disorder that exacerbates the symptoms related to terminal illness.
5. Assesses the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect
6. Assesses environmental resources and obstacles to maintaining safety
7. Participates in the development and revision of the plan of care
8. Provides social services including:
a. Short-term individual counseling
b. Crises intervention
c. Assistance in providing information and preparation of advance directives
d. Funeral planning issues and transfer of responsibilities regarding fiscal, legal and health-care decisions
9. Prepares clinical and progress notes
10. Identifies family dynamics and communication patterns
11. Involves the patient/family in the plan of care
12. Identifies and utilizes appropriate community resources and assesses patient/family ability to access them
13. Evaluates patient/family response to psychosocial interventions
14. Assesses caregiver’s ability to function adequately
15. Assesses caregiver’s ability to function adequately
16. Assess need for counseling related to risk assessment for pathological grief
17. Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions
18. Identifies the developmental level of patient/family and obstacles to learning or ability to participate in care of patient
19. Addresses patient/family questions and issues
20. Identifies obstacles to compliance and assists in understanding goals of interventions
21. Identifies support systems available to reduce stress and facilitate coping with end-of-life care
22. Evaluates for long-term care when appropriate and assesses ability to accept change in level of care
23. Communicates psychosocial information in inpatient facility when level of care is changed
24. Assists patient/family in assessing financial resources when appropriate
25. Identifies patient/family needs when discharged or when level of care is changed
26. Assists patient/family needs when discharged or when level of care changes
27. Evaluates patient/family/response to intervention(s) when referred to community agency and satisfaction of the service(s) provided
28. Provides support and direction to team members faced with inter-family or inter-team conflict
29. Assesses the bereavement needs upon initial evaluaton with a Pre-Bereavement Assessment.
30. Participates with interdisciplinary team and is often the team leader.
31. Participates in staff development activities and in-service education.
32. Assumes responsibility for self-development by continually striving to improve his/her medical social worker practice through formal education, attendance at workshops and conferences, active participation in professional and related organization meetings, individual research, and reading.
33. Supervises the social work assistant (LBSW) as indicated.
34. Performs other related functions as requested.
Driving & Other Requirements
Must have and maintain a valid state driver license, have a registered and reliable vehicle that meets state law standards, and meets the insurance standards of company. Position requires travel between agency site and clients’ home as well as the community. Traveling by car or airplanes to local or out-of-town meetings, seminars, and conferences limited.
