To ensure a safe and supportive environment for all patients, visitors, and volunteers, the following items are required as part of the application process. Please review carefully before submitting your materials.
Medical Insurance Documentation
All volunteer applicants must provide a copy of their medical insurance card. This is required for participation in the Mid-Valley Hospital Volunteer Program. Applications submitted without proof of medical insurance cannot be processed.
Volunteer Interest Statement
Please include a separate page that outlines:
- Where within Mid-Valley Hospital you would like to volunteer
- Why you are interested in volunteering in that department or role
This information helps us determine placement and ensures a meaningful experience. Applications missing this statement will not move forward.
Minor Applicants (Under 18)
Additional documentation is required for minor applicants, including:
- Parent or legal guardian consent
- Any other age-related forms required by the hospital
