Program Description: DDI Vantage empowers vulnerable children and families to reach their full potential. Serving the community since 1971, this non-profit organization offers Early Intervention, Early Head Start Home-based services and Early Head Start Child Care Partnerships. 


Position Description: The full-time (40-hour/week) Family Service Coordinator II coordinates program services provided by the early intervention team, supports and advocates for families and children, and ensures compliance by monitoring written documentation.


Responsibilities:

  • Makes the initial family contact and gathers information to identify family concerns, priorities, and resources. 
  • Educates families of their rights and responsibilities.
  • Completes Concerns, Priorities, and Resources (CPR) CPR pages.
  • Schedules evaluations and assists the evaluation team in determining program eligibility.
  • Ensures compliance with the Individualized Family Service Plan (IFSP).
  • Coordinates educational, community, and social services resources for families based on the IFSP.
  • Assists in identifying social service needs for families and refers them to the program director for help in accessing community resources when appropriate.
  • Coordinates upcoming due dates for six month reviews, annual assessments, and transition reports; and schedules and coordinates regular evaluations, IFSP meetings, transition meetings, and closure meetings.
  • Ensures a seamless transition of children from DDI Vantage to other agencies.
  • Assists in child-find activities. 
  • Attends all mandatory  Comprehensive System of Personal Development (CSPD) training for early intervention credentialing.
  • Completes written documentation in a timely manner to ensure program compliance.
  • Demonstrates professionalism in all aspects of the job.

Qualification Requirements

  • Education/Experience: The Family Service Coordinator II must have a bachelor’s degree from an accredited college or university in a related field. Experience working with infants and toddlers is a plus. 
  • A state and federal background screen and motor vehicle records check will be conducted on all new hires.
