Now Hiring - Advanced Practice Nurse/Interna Medicine/General Practice NP's 

Multiple opportunities available - Boston South; Metro Boston; Boston North; Western MA

 

Hours: 

 

The Advanced Practice Nurse will be responsible for, but are not limited to:

  1. Conduct visits to patients on palliative care in accordance with local, state and federal regulations.
  2. Communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care.
  3. Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment).
  4. Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
  5. Prepare and maintain accurate patient records, charts and documents.
  6. Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed.
  7. Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Education:

Master's Degree in Nursing

 

Professional Experience:

  • Must have a Nurse Practitioner's license in the state of operations
  • Must have, at minimum, ONE year experience in the capacity as a Nurse Practitioner in a hospital, skilled nursing or acute care setting (or similar field)
  • Board Certified in Internal Medicine, or General Practice
  • Must have a Master's Degree
  • Must have a valid driver's license and a reliable source of transportation
  • Must be willing to travel locally
