Under the general supervision of the PACE Transportation Manager and Director, plans, organizes and implements the transportation services of the day health center.
Location: Modesto, Ca.
Schedule : Monday - Friday, 8:00am - 5:00pm.
Compensation: $33.61 – $35.29 an hour
Duties and Responsibilities
- Plans and coordinates all scheduling activities related to transportation for Central Valley PACE participants.
- Supervises daily, the activities of the coordinator, drivers and aides.
- Responsible for coordination of all outside service providers of transportation.
- Reviews and verifies all bills for services provided by outside contractors.
- Assures all necessary program records are maintained according to policy.
- Participates as a member of the Interdisciplinary Team (IDT). Attends staff meetings and takes part in participant care planning.
- Coach and counsel staff and administer appropriate disciplinary action in a timely manner.
- Participating in performance reviews, monitoring budgetary practices, preparing staff schedules, developing and implementing performance improvement activities and providing off-hours administrative and in-person support.
- Conduct system-wide analysis of transportation operations to ensure efficiency.
- Coordinate resources across staff and contracted vendors and engage with vendors based on contractual thresholds, and reprioritize participant trips.
- Maintaining and communicating policies and procedures clearly.
- Makes adjustments to routes/trips on short notice and communicate changes.
- Investigate participant/caregiver complaints and escalate to the quality team when necessary.
- Model and promote service excellence and respectful relationships with participants, caregivers, and staff.
- Observe each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.
- Assists passengers on and off the van in a safe and courteous manner. Will work with medical equipment and manage other emergencies that may arise.
- Ensures vehicle inspection logs are maintained and required maintenance completed.
- Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
- Trains drivers on established processes and test for competencies.
- Participates in continuous improvement activities and use lean tools to measure results.
- Serves as a backup driver as necessary.
- Communicates scheduling to essential transportation team members to ensure all routes are appropriately scheduled.
- Other projects and duties as assigned.
Physical Demands
- Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate.
- Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
- Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
- Must have high manual dexterity.
- Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.
Work Environment
- Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
- Subject to unpleasant odors.
- Exposed to outdoor elements of heat, cold and rain when transporting patients.
- The noise level is usually quiet to moderate, but may at times be noisy and crowded.
Education/Experience Requirements
Minimum Qualifications
- Valid CA Driver's License, clean driving record, and vehicle insurance.
- Current BLS CPR Card, certified by the American Heart Association or American Red Cross.
- Upon hire, Defensive Driving Course will be required.
- Must have good interpersonal skills and be able to work effectively in a team environment.
- Meet a standardized set of competencies for the specific position description established by Central Valley PACE and approved by CMS before working independently.
Education/Experience
- High School Diploma, GED, or equivalent required.
- Minimum two (2) years supervisory experience required, within a transportation setting preferred.
- Minimum one (1) year routing/dispatching experience.
- Minimum two (2) year experience transporting individuals with special mobility needs.
- Six Sigma certification, preferred.
- One (1) year prior personal care assistance experience, preferred; or ability to pass internal competencies.
