The Qualified Intellectual Disabilities Professional (QIDP) functions as an advocate, a facilitator and Case Manager in conjunction with the Interdisciplinary Team, overseeing the development and implementation of his/her assigned individual's program plan relative to active treatment issues and makes recommendations and requests on behalf of the individuals for needed services and interventions.

Schedule: Monday – Friday, standard business hours (80hrs biweekly)

Salary: $55,702.40/annually

Job Responsibilities

  • Participates in the intake and admission process of new individuals.
  • Functions as the Plan Lead in the development, implementation, and monitoring of each individual’s program plan, reviewing annual updates, coordinating and attending the annual meeting.
  • In collaboration with the Interdisciplinary Team (IDT), supervises, monitors, and evaluates services, equipment, and programs that are needed to meet each individual’s needs and ensures active treatment.
  • Coordinate the completion of the initial assessment for each individual on their case load, and each subsequent assessment thereafter.
  • Monitors the participation and progress of outcomes outlined in the individual’s program plan by reviewing the documentation of goals, at least monthly.
  • Review all documents and quarterly reports on progress of goals outlined in plan.
  • Informs the Plan Team, including the Supports Coordinator, of the individual’s progress on their goals and outcomes as applicable, changes in the individual’s needs recommending a revision to an outcome or services as needed and informs them of their right to decline the IPP review documentation.
  • Consults with personnel from other agencies, when necessary, for evaluations and other services.
  • Participates in the development of behavioral support plans.
  • Maintains accurate and up to date case notes for each individual on their caseload.
  • Assist with preparation for waiver submissions.
  • Participates in the Certified Investigators training and becomes certified within a year (as training availability permits.
  • Other duties as assigned. 

Requirements

  • Bachelor’s Degree required.
  • 2 years experience working with individuals with Intellectual Disabilities.
  • 1-2 years QIDP experience (preferred).
  • Working knowledge of MS Word, Excel, Outlook, and basic knowledge of Access and/or ability to use & learn various database reporting systems.
  • A valid Driver's License.
