Description
The Case Manager is a critical position of Morris House’s functioning and overall success of our clients. The Case Manager is responsible for providing recovery- orientated services to individuals with substance use disorders and/or co-occurring disorders that are in need of routine or intermittent low-intensive support needs, including health needs while residing at The Morris House, a 90-day residential Recovery House. The Case Manager shall assist individuals with achieving an optimal quality of life while transitioning successfully into the community. Case Managers are usually assigned a caseload that consists of 60% direct service time and 40% indirect service time. Direct service includes assessment interviews, individual, family and group education and/or resource support and discharge planning. Indirect service time includes supervision, staff meetings, telephone calls, treatment authorizations and all paperwork associated with client care.
EXAMPLES OF RESPONSIBILITIES:

Collaborate with individuals to identify service and support needs. Conduct intake assessments that help individuals identify and explore personal strengths and community recovery resources and supports and develop recovery plans that address needs in areas such as employment, education, self-management skills, relapse prevention and social skills training.

Responsible for assisting individuals to access services and supports that enhance independent participation in social, interpersonal, family, and community activities. Help develop self-advocacy skills for the purpose of accessing natural supports, self-help, and other advocacy resources. Assist client is accessing health and wellness education and services.

Facilitate Psycho-educational groups on topics related to life skills training, family unification, parenting skills, job search skills, anger management, relapse prevention, stress management, and support for spiritual/religious involvement.

Work with and advocate for individuals in the community setting. Such as accompanying clients to appointments, court or other legal appearances, housing searches, and/or obtainment of basic needs.

Responsible for working with the client to develop a discharge plan and confirm referrals to appropriate aftercare services in collaboration with the referral source based on individual’s needs.

Assist with residential components of the program to ensure client safety. Including but not limited to; client counts, supervised smoke breaks, meal prep, room check/searches and supervision of client down time.

Responsible for supervision of Self-Administration of Medication process and supervision of drug screening while on shift.

Represents the agency at community outreach events and functions.

Responsible for maintaining documentation in compliance with all agency policies and procedures.

Develop and maintain positive working relationships with management/leadership, coworkers, referral sources, other community providers and clients utilizing a person centered and team approach.

Coordinate with treatment team as needed to ensure that clients and their needs are addressed as part of the holistic, whole-person, gender responsive, culturally sensitive integrated care approach.

Participate in supervision, meetings, agency committees and work groups as assigned by management. May be required to participate in trainings (both on site and off site) as determined by the Director of Outpatient and/or Vice President of Adult Services.

Qualifications
EXPERIENCE/EDUCATION:
Associate degree with experience in the field of substance use and dual diagnosis or Certified Alcohol and Drug Counselor (CADC) preferred.
Candidate must be flexible, with a high degree of motivation, exercise good judgment, and be able to work independently. Solid computer skills are required. Interest in working with a multi-disciplinary team is strongly encouraged. Training in Motivational Interviewing and Mental Health First Aid is a plus.
Valid driver’s license with clean DMV driving record, reliable transportation and valid auto insurance.
CPR/first-aid certification must be obtained and maintained.
