Work Location/Program Overview:

The Community Supports Navigator provides support, assistance and instruction to individuals with a developmental disability (ID/DD) and/or autism spectrum disorder (ASD) living independently or at home with a legal guardian. Examples of support include but are not limited to: assistance with apartment maintenance, money management, medical appointments, advocacy, meal preparation and nutrition, socialization, crisis intervention and skill acquisition. The Case Worker is also responsible for providing quality documentation, support of corporate rep-payee responsibilities and may be asked to provide surrogacy supports.

Position Overview:

• Responsible for creation and implementation of ISP, Behavior Plans, Medication Treatment Plans, Assessments, and any other applicable documentation related to successfully supporting individuals in the community within designated time frames
• Provide training, modeling and assistance in activities of daily living and health maintenance in order to enhance skill acquisition and maintain independent living. These may include but are not limited to; meal preparation and nutrition, apartment maintenance, exercise, socialization and utilizing public transportation/ADA.
• Provide case management for individuals including support, assistance and instruction to develop independence and achieve goals set in ISP.
• Provide training, modeling and assistance in activities of daily living and health maintenance in order to enhance skill acquisition and maintain independent living. These may include but are not limited to; meal preparation and nutrition, apartment maintenance, exercise, socialization and utilizing public transportation/ADA.
• Work collaboratively with Ascentria Leadership and Finance to ensure compliance with all rep payee responsibilities such as but not limited to: Financial Training Plan, Funds Management Plan, annual reporting, and bill payment
• Provide transportation for and escort individual to and from appointments as needed.
• Assist in obtaining public benefits such as but not limited to: Social Security, Mass Health, ADA transportation, food stamps and fuel assistance.
• Assist individuals when moving from one location to another. This may include but is not limited to: moving furniture and/or boxes and using personal vehicle to transport client personal belongings.
• Provide individual with basic problem solving information. Model and teach appropriate social behavior.
• Serve as an advocate for individuals at various community levels, evaluate effectiveness of advocacy efforts and make appropriate adjustments.
• Provide regular updates and communication with Program Manager, Clinical Coordinator, and DDS staff.
• Prepare required paperwork and submit by deadline.
• Attend and participate in staff, team and PQI process meetings.
• Job must be conducted at all times in a way that respects the rights, privacy and the dignity of the client and adheres to all contractual regulations.
• Represent Ascentria Care Alliance in the community with professionalism and appropriate business attire.
• Perform other duties as assigned.

Here’s what we’re looking for:

• Bachelor’s Degree Required with a minimum of 1 year experience in human services related work
• HS degree with a minimum of 10 years experience in human services direct care work may be considered as an alternative
• Licensed or eligible to be licensed by ASWB according to education and experience level (LICSW, LCSW, LSW or LSWA) within 6 months of being hired strongly preferred
• Minimum of two (2) years of experience in human services required, preferably in community based settings.
• Demonstrated knowledge of the DDS ISP process and Charting the Life Course is strongly preferred
• Ability to implement treatment plans and set limits and manage client behaviors.
• Ability to write accurate and professional documentation in legible English on a daily basis.
• Ability to set priorities and manage multiple tasks to deadline.
• Ability to work independently and in a team environment with community based human service professionals and DDS staff.
• Ability to work independently and as part of a multi-cultural team is required
• Basic Microsoft Office computer skills.
• Flexible schedule to meet the needs of individuals
• Provide crisis intervention in a calm and effective manner as needed.
• Excellent verbal and written communication skills.
• Ability to work within the Mission of the agency, “we are called to strengthen communities by empowering people to respond to life's challenges.”
• Valid Driver’s license and proof of automobile insurance is required to transport clients throughout the community.
