POSITION SUMMARY:
The Program Specialist is responsible for supporting the health, safety, and well-being of individuals with disabilities in both facility-based and community settings. This role promotes independence by encouraging the agency’s adult training program participants to live within the community and learn
employment skills to promote independence. The Program Specialist will manage the program’s participant caseload, develop personalized goal plans and coordinate services that reflect each individual's unique needs, preferences, and aspirations. This role requires knowledge of electronic data systems for case management documentation and tracking purposes.
Duties include but are not limited to:
- Complete assessments by collecting information about the participant to identify individual needs from members of the interdisciplinary team.
- Participate in the individual service plan (ISP) development process, team reviews and implementation of the ISP in accordance with licensing, accreditation, and regulatory requirements.
- Maintain files in accordance with licensing requirements.
- Ensure participants understand their individual rights through day-to-day interactions, service meetings and verbal reviews.
- Conduct progress reviews of individual’s goals in accordance with program and regulatory guidelines.
- Provide and supervise activities for participants in accordance with the individual plans and, as needed, support the integration of participants in the community.
- Provide coverage when needed in the facility, community and for virtual services.
- Support participant communication and involvement with families and friends.
- Utilize current technology and software to provide service to individuals in programming.
- Adhere to agency and Office of Developmental programming incident reporting and investigation procedures. Assist with de-escalation and caregiver correspondence throughout.
- Uphold confidentiality in compliance with HIPAA and agency expectations, while providing professional and empathetic communication to participants, families, and support teams.
- Participate in the program’s Advisory Council to foster family caregiver and stakeholder relationships.
Schedule: Monday – Friday (8:00am – 4:30pm). Can vary depending on department needs.
External Hiring Range: $20.08-$20.88/hour
QUALIFICATIONS:
- Associates' Degree (Psychology, Social Services, or related) AND 4+ years' of required experience OR Bachelors Degree AND 2+ yeas' of required experience OR Masters Degree AND 1+ years' of required experience
REQUIRED EXPERIENCE:
- Successfully working with individuals with disabilities/barriers to employment
- Proficiency with case management software and data entry
PREFERRED EXPERIENCE:
- Prior experience working in an adult training facility
- Knowledge of 2380 licensing and program regulations
REQUIRED CLEARANCES, LICENSES, AND CERTIFICATIONS:
- Candidates are expected to provide current valid clearances (Child Abuse Clearance, FBI Fingerprints Clearance, and PATCH) prior to first day of employment.
- Must have a valid driver’s license and reliable transportation.
- Cardiopulmonary Resuscitation (CPR), First Aid, and Automated External Defibrillator (AED) certifications required within the first 90 days of employment.
