The Care Manager working jointy with the Health Services Director (Licensed Practical Nurse) is responsible for coordinating and overseeing the delivery of quality resident care and clinical services within the assisted living community. This role ensures compliance with state regulations, community policies, and best practices while fostering a caring and supportive environment for residents, families, and staff.

Key Responsibilities
Resident Care & Clinical Oversight
-
Conduct and oversee initial and ongoing resident health assessments and service plans.

Monitor changes in resident conditions and ensure appropriate clinical interventions are implemented.

Administer medications and treatments per physician orders and in accordance with state regulations.

Coordinate and supervise care provided by nursing assistants and caregivers.

Maintain accurate and timely documentation of resident care, assessments, and communication logs.
Leadership & Supervision
-
Provide leadership, training, and support to care staff to ensure consistent delivery of quality care.

Schedule, supervise, and evaluate care team performance, promoting teamwork and accountability.

Assist with recruitment, onboarding, and retention of care staff.
Compliance & Quality Assurance
-
Ensure compliance with all regulatory, licensing, and safety requirements.

Participate in audits, quality assurance initiatives, and resident care conferences.

Maintain confidentiality and adhere to HIPAA guidelines.
Resident & Family Relations
-
Serve as a primary point of contact for residents and families regarding health concerns and care plans.

Support residents’ rights, dignity, and individuality in all aspects of care.

Communicate effectively with families, physicians, and interdisciplinary teams.
Administrative Duties

Maintain accurate resident records, medication administration records (MARs), and physician orders.

Participate in community meetings, safety drills, and staff training programs.

Assist the Health Services Director with reporting and compliance documentation.
Qualifications
Qualifications
-
Education: Graduate of an accredited nursing program; current state LPN license in good standing.

Experience: Minimum 1–2 years of nursing experience, preferably in assisted living, long-term care, or geriatric settings.
