myPlace Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. We specialize in providing value-based, comprehensive care and coverage for older adults with significant needs so they can thrive in the homes they love and in the communities they cherish.
Our mission is simple: to enable older adults to live the independent lives they deserve. We pursue this mission through our myPlace PACE (Program of All-Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant’s preferred place.
myPlace Health is building a mission-driven team that shares our passion for redefining the way older adults experience care as they “age in place” in the community. This is a unique opportunity to take on one of our country’s most challenging healthcare problems and join a fast-growing, dynamic team as we prepare to scale our mission to serve more markets.
About this Role
If you’re passionate about helping older adults live safely and comfortably at home, this role is for you. As a Home Care Coordinator (LVN), you’ll play a vital role in connecting participants to the compassionate care they deserve—whether that’s through trusted home care partners or your own hands-on support.
You’ll work closely with our Interdisciplinary Team (IDT) to design thoughtful care plans, coordinate visits, and ensure every detail is captured in our EMR system. Each day brings new opportunities to make a meaningful difference and build lasting relationships that help our participants thrive in their communities.
Join us in creating a more connected, caring model of healthcare—one participant at a time.

Team-Oriented: You work effectively as part of a multidisciplinary team, collaborating with physicians, nurses, and other healthcare professionals. You actively contribute to care plans, assessments, and communication, always with the goal of improving participant outcomes.
Excellent Communication Skills: You excel in both written and verbal communication. You document accurately in EMR systems, and you’re skilled at conveying important health information to team members and participants' families.
Adaptable and Flexible: You’re comfortable working in dynamic environments, whether in participants' homes, in the clinic, or in collaboration with outside agencies. You can adjust to varying needs and priorities with a positive attitude.
Organized and Efficient: You keep accurate records, stay on top of appointments, and manage your time efficiently to ensure care is delivered on schedule. Your attention to detail ensures that nothing falls through the cracks.
Compassionate and Caring: Your kindness and genuine care shine through in your interactions with participants. You go beyond just the physical aspects of care, offering emotional support and a sense of security to those you serve.
The Fine Print
You’ll be on the move: This role is up to 100% in the field, visiting participants in their homes and throughout the community to ensure they receive personalized, high-quality care.
