Rehabilitation Supervisor – PACE (Program of All-Inclusive Care for the Elderly)
Location: Riverside, CA
Schedule: Full-time, Monday–Friday, 8:00 AM – 4:30 PM (On-site)
Compensation:
PT/OT: $104,200–$156,200 annually
PTA/COTA: $80,200–$115,500 annually

About the Role
The Rehabilitation Supervisor oversees daily operations of rehabilitation therapy services within a PACE (Program of All-Inclusive Care for the Elderly) setting. This position manages a multidisciplinary therapy team and ensures participants receive high-quality, compliant, and coordinated care. The ideal candidate is an experienced therapist or therapy assistant who thrives in leadership, patient-centered care, and interdisciplinary teamwork.
Key Responsibilities
Supervise licensed therapists and therapy assistants, ensuring continuity and quality of participant care
Develop, implement, and monitor individualized therapy treatment plans
Lead onboarding and training for new staff on PACE and CMS compliance
Oversee therapy policies, procedures, and best practice development
Conduct participant assessments to determine mobility, function, and independence goals
Guide patients and caregivers in adaptive equipment use and self-care techniques
Maintain compliance with PACE, CMS, and state regulations
Oversee scheduling, case management, and therapy documentation
Facilitate team meetings, care planning sessions, and departmental training
Evaluate treatment effectiveness and modify care plans as needed
Manage therapy equipment and supply needs for the program
Leadership & Supervision
Lead a therapy team focused on high-quality, coordinated participant care
Support professional growth and performance management for staff
Participate in quality improvement initiatives and program development
Collaborate with interdisciplinary teams across departments and locations
Ensure departmental compliance with safety and privacy standards
Qualifications
Valid California license as a Physical Therapist or Occupational Therapist
ORGraduate of an accredited PTA or COTA program with active California license
Current BLS certification (AHA) required upon hire
Minimum 2 years of therapy experience; 1 year working with a frail or elderly population
Supervisory experience preferred; PACE experience highly desirable
Strong knowledge of geriatric therapy practices and interdisciplinary collaboration
Excellent communication, organizational, and problem-solving skills
Ability to travel between sites within the region
