Clinical Supervisor RN

Responsibilities:
- Delivers direct patient care including initial and on-going assessments of the patient
 - Develops and implements the Hospice Plan of Care in conjunction with the other members of the Interdisciplinary Team (IDT)
 - Provides education to patient/family on the hospice philosophy and care of the terminally ill patient and the patients� individualized plan of care.
 - Works with Administrator/Director of Nursing to ensure appropriate staffing levels are maintained according to policy
 - Maintains primary control and management of each patient and acts as the primary liaison between physician, patient/family and the IDT
 - Supervises all patient care activity to assure compliance with current standards of accepted nursing and medical practice.
 
Qualifications:
- Minimum of two (2) years full-time experience as an RN, either in oncology, an acute care setting with direct patient care, a long-term care facility, hospice or home health care.
 - Current license to practice nursing in the state of Louisiana
 - Current CPR certification required
 - Home Care Home Base experience preferred
 - Current automobile insurance and valid driver�s license, maintain transportation to be used for work
 - Excellent patient assessment skills.
 - Good oral and written communication skills with an empathetic and people-oriented personality
 - Knowledge of managed care principles, and regulatory guidelines (i.e., Medicare, Medicaid, TJC, ACHC)
 - Proficient with technology and willing to learn new systems
