Under the direct supervision of the Site Director and alongside the Interdisciplinary Team (IDT), the Care Plan Coordinator is responsible for scribing, documenting, coordinating and implementing the participants plan of care. The Plan of care is a living document that is regulatory required by CMS, Center for Medicare Services and DMAS, Department of Medicaid Assistance Services.
Responsibilities
Participation in all IDT meetings.
Send letters and scheduling all care plan meetings.
Scribes IDT meeting agendas.
Ensure compliance of CMS regulations with all care plan content and deadlines.
Document the development of care plans and reviews with participants.
Maintains all care plans to include relevant updates.
Documents all IDT and outside recommendations.
Documents and tracks all Service Determination Requests and Grievances.
Coordinating family and caregiver meetings.
Communicates daily with the IDT on any issues, needs, or updates.
Performs other duties as assigned.
Qualifications
Required Qualifications:
Required Education: High School or GED
Required Experience: At least 2 years of experience of healthcare documentation and
collaboration with a treatment team.
Must have experience of working with older adults.
Experience with Excel and Word documents
