As a Social Worker, provide medically related social services that assist the residents to attain or maintain the highest practicable physical, mental, and social well-being.

Social Worker Director Key Duties
- Conducts pre-admission screening of individuals seeking admission to the facility, including persons with mental illness according to facility admission criteria.
- Completes and updates social services assessments for residents.
- Function as Care Plan Coordinator as assigned.
- Works cooperatively with members of the interdisciplinary team to develop, implement and evaluate plans of care to protect and promote residents’ rights.
- Records progress notes in the clinical record. Observations of resident’s behavior and interventions
- Perform other job-related duties as they become required.
Social Services Director Qualifications
- Licensed Social Worker preferred.
- Two plus years experience with Seniors in healthcare setting. Skilled long-term care is preferred.
