SUMMARY OF THE POSITION: 

A Client Supports Manager oversees implementation of client plans and ensures quality care through training of direct care staff, staffing required supports, and coordinating care among client team members. A Client Supports Manager is also responsible for promoting Tangram’s Core Values and mission. 

Leadership Responsibility

· Advance Tangram’s mission, strategic plan, and values.

· Provide direction, leadership, performance feedback, and training for direct reports and direct support staff.

· Build positive relationships with clients, teams, professionals, and members of the community. 

· Participate in ongoing professional development and promote continued development of team members.

· Builds a strong team of direct support staff to meet client needs.

· Facilitate positive relationships with natural supports for persons served.

· Looks for opportunities to add clients and expand services.

Duties/Responsibilities:

· Monitor implementation of client plans, outcomes, activity notes, and report progress.

· Ensure health and safety of clients including review of medication administration, follow-up on medical appointments, emergency planning, site inspections, and emergency drills. 

· Ensure accurate service allocations, utilization of resources, and documentation.

· Obtain and/or complete all admission, enrollment, and training documents for clients.

· Participate and help guide Person-Centered Individualized Support Team (PCISP) team meetings.

· Create meeting notes and Quarterly Reports for client meetings.

· Maintain client records such as financial data and medical appointments. 

· Report critical issues, incidents, and emergent needs.

· Visit clients receiving residential supports, in person, at least once weekly. 

· Provide direct support for clients as needed including transportation to appointments, community engagements, and personal care. 

· Oversee and implement entitlement systems as it relates to Social Security, Medicaid, Medicare, and Section 8 housing, providing needed documentation. 

· Maintain compliance with service standards and accreditation requirements. 

· Coordinate with Human Resources the posting and recruitment of open positions.

· Responsible for training, coaching and performance management for assigned personnel, which includes the timely completion of performance reviews. 

· Manages direct report hours in the most cost effective manner. 

· Works an average of 6 hours of direct support per week to help enhance skills in daily life and employment, healthy living, social connection, advocacy, or other important life domains.

  • Participates      in an on-call rotation. 
  • Other      duties as assigned. 


Requirements

 Required Skills/Abilities:· Strong leadership qualities including excellent communication skills, ability to provide on-call support in a 24-hour setting, demonstrates strong ethical standards, and promotes a culture of learning and growth.

  • Must      maintain CPR and first aid certification. Must pass Medication      Administration and must stay current on all other training as defined by      state, accreditation body or agency guidelines. 
  • Working      knowledge of Microsoft products.

Education and Experience:

  • Bachelor's degree in rehabilitation, psychology,      or related major. Consideration may be given to extensive experience and      extraordinary performance in lieu of degree.

· Three years of experience in the human services field with one year of supervisory experience. · Must be 18 years of age or older, must have a valid driver’s license, acceptable driving record, meet agency’s driver insurability requirements, reliable vehicle, and up to date automobile insurance. If position involves driving agency vehicles, must be 21 years of age. Must have basic computer skills.Physical Requirements:

  • Prolonged periods of sitting or working on a computer.
  • Ability to lift up to 50 pounds.
  • Repetitive physical activity such as data entry.
  • Must be able to lift/transfer individuals and physically assist clients with movement and care.
