8618 Executive TeamThe Vice President of Population Health is responsible for the following departments/programs: Rural Health Clinics, Urgent Cares, Health Homes, and Care Transformation Programs. With direction from the CEO, the Vice President will oversee, direct and coordinate clinic operational activities and as well as be responsible for the development, implementation, monitoring and management of population health programs in collaboration with health plans, grantors and local, state and federal agencies. The Vice President will exercise responsibility and serve as a leader in driving clinical transformation, delivering a comprehensive interdisciplinary approach to achieve care delivery excellence throughout the patient care continuum. These changes measurably improves quality, creates holistic, patient centered care experiences, improves provider and staff satisfaction and reduces healthcare costs by eliminating waste and optimizing the value proposition enabling the organization to successfully enter into alternative payment models of the future.Job Description
The Chief Ambulatory Officer (CAO) is responsible for the following departments/programs: Rural Health Clinics, Primary Care Medical Clinics, Urgent Cares, Specialty Care Clinics and Population Health Programs. With direction from the CEO, the CAO will oversee, direct and coordinate clinic operational activities and as well as be responsible for the development, implementation, monitoring and management of population health programs in collaboration with health plans, grantors and local, state and federal agencies. The CAO will exercise responsibility and serve as a leader in driving clinical transformation, delivering a comprehensive interdisciplinary approach to achieve care delivery excellence throughout the patient care continuum. These changes measurably improve quality, create holistic patient centered care experiences, improve provider and staff satisfaction and reduce healthcare costs by eliminating waste and optimizing the value proposition enabling the organization to successfully enter into alternative payment models of the future.
Compensation Range: $375,000 to $475,000
QUALIFICATIONS
Education
Required: Master’s degree in Nursing, Health Care Administration, Business, or related discipline
Experience
Required: Minimum of ten years of experience at the Senior Management Level of a medium-sized or large healthcare organization
Leadership experience in outpatient clinics (i.e. rural health clinic, federally qualified health center and 1206-D hospital licensed clinics) Health plan contracting, quality incentive programs and alternative payment models (accountable care organizations (ACO), global risk, etc.) Quality reporting (UDS, HEDIS, CMS Stars, MIPS, etc.) Physician and medical group recruitment and contract negotiations
JOB RESPONSIBILITIES
Essential
Provides leadership, direction, and coordination to the outpatient clinic network and population health initiatives.
Responsible for integrating the strategic plan of the organization within the population health division with a focus on High-Performing Outpatient Delivery Network initiatives.
Provides executive level oversight for the development of high quality, cost effective programs that align and efficiently integrate quality initiatives with operations.
Develops and implements strategic plans to accurately forecast revenues, expenses and capital requirements for service lines.
Articulates a clear vision and new business strategies to enhance market share and improve the overall performance of the areas of responsibility.
Works with the marketing team to market the service lines to physicians and the community to achieve projected volumes and market growth.

Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area.
