First Choice Health Centers is seeking a compassionate and dedicated Registered Dietitian to join our interdisciplinary team. We provide comprehensive outpatient services with a focus on preventive care, chronic disease management, and equitable access to health resources. We are looking for a Registered Dietitian to join our multidisciplinary team and help patients achieve sustainable, healthy lifestyles through individualized nutrition counseling and education. This position is located at 809 Main Street, East Hartford, CT with opportunity for hybrid work.

• Schedule is Monday, Wednesday, Thursday 8:00 am to 4:30 pm, Tuesday 8:00 am to 6:00 pm and Friday 8:00 am to 3:00 pm
• The annual pay range for this position is $65,000 - $80,000. Pay is based on several factors including but not limited to work experience, education certifications, etc.

Minimum Knowledge, Skills & Abilities Required:

• Requires bachelor's degree in dietetics, nutrition, or closely related field
• Active state of Connecticut license as Dietitian/Nutritionist
• Dietitian/Nutritionist certification in CT
• Commission on Dietetics registration
• Ability to work as part of multi-disciplinary team
• Desire to serve a diverse population in a community health setting
• Home address must be within commuting distance of East Hartford, CT

Experience and Training:

• 3 years' experience in a position with duties involving dietetics and/or nutrition

Standard Job Duties:

• Conduct comprehensive nutrition assessments for patients with conditions such as diabetes, hypertension, obesity, gastrointestinal disorders, and cardiovascular disease.
• Develop individualized nutrition care plans and monitor progress toward goals.
• Provide culturally sensitive counseling and education to patients and families
• Collaborate with primary care providers, behavioral health specialists, and care coordinators to ensure coordinated care.
• Document all encounters in the electronic health record (EHR) in accordance with clinic and regulatory standards.
• Participate in community outreach events, health fairs, and wellness programs.
• Stay current with evidence-based nutrition practices and participate in continuing education.
