As a Resource CNA, you are a member of the dedicated CNA team that is responsible for supporting the team in covering assignments in providing personal care services to the terminally ill hospice patient as needed under the direction of the RN. The Resource CNA will also provide support for new and established aides with guidance and education in their perspective role as defined by the Capstone processes and IDG care plan.
Our CNA team provides quality care and support to our patients who are in the final stages of their life while providing understanding and emotional support to the families. Capstone provides hospice services to patient in their private homes and senior living communities.
One Resource CNA will be assigned to a defined county of the Capstone service area. One candidate per county will be considered for the position. The counties are: Dekalb, Fulton, North Gwinnett & West Gwinnett & Cobb County.
This position will have the expectation of working Monday - Friday, no evenings and no weekends. Daily driving to, between and from patients' location, reliable transportation is required, eligible for mileage reimbursement.
Requirements
Candidates must possess the following qualifications and experience to be considered:
- High school diploma or GED.
- Graduate from an approved training competency course, registered Nurse Aide with the State of Georgia.
- Meet federal Home Health Aide training and competency requirements.
- Minimum of solid 2 years' experience as a nursing assistant.
- Active patient contact within the past three years, preferred.
- Proven and dependable career work history as a CNA.
- Competent in electronic documentation.
- Must have reliable transportation.
To be successful in the role, candidates must possess the following knowledge, skills and ability:

- Ability to respond appropriately to crisis outside of a hospital setting.
- Acceptance of caring for patients in their personal homes.
- Ability and experience using an electronic medical record system.
- Compassionate and empathetic nature
- Ability to make accurate, and at times, quick judgments.
- Excellent communication and interpersonal skills
- Ability to respond appropriately to crisis outside of a hospital setting.
- Acceptance of an adaptability to different social, racial, cultural and religious modes.
Essential Responsibilities:
- Under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to the patient:
- Bath, care of mouth, skin and hair.
- Bathroom assistance while using bedpan or commode.
- Bed transfers as well as ambulation activities as ordered under the care plan.
- Range of motion exercises which the Aide has been taught by appropriate professional personnel.
- Performing such incidental household services as are essential to the patient’s well-being at home.
- Preparing and assisting with meals, including special diet.
- Provide emotional support and compassionate care to the patient and family unit.
- Demonstrate listening skills.
- Offer encouragement when appropriate.
- Practice patience in voice and manner
Maintains the required competency and skills:
- Documented initial and ongoing every 14-day supervision and evaluation by RN on each case assignment.
- Participates in annual performance review.
- Reports any changes in the patient's condition or family situation immediately by calling the Hospice office and notifying the RN.
- Practices Standard Precautions according to OSHA regulations and policy.
- Adheres to all policies procedural guidelines.
- Attends at least twelve (12) hours of in-service education programs a year.
- Maintains current certification as required by the State.
- Maintains current knowledge and practice of infectious disease protocols.
Physical Requirements:
Requires considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position. Must provide evidence of annual TB test and other state-required tests or examinations.
Mental Requirements:
Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.
Working Conditions:
Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials.
Transportation:
Must have a current valid driver’s license, auto liability insurance and reliable transportation.
This is a full-time hourly non-exempt position, eligible for overtime.
