Responsible for assessing patients' psycho-social needs and counseling services to individuals, families, and groups. Provide direct services to clients and complete associated documentation for the client and program.
Responsibilities
- Screens individuals for substance abuse, including physical and mental health status.
- Performs biopsychosocial intakes and assessments.
- Develops treatment plans, including measurable goals and objectives.
- Provides individual and group counseling, and educational programs in accordance with the treatment plan.
- Documents treatment and discharge plans, and clients’ progress and responses to treatments; and maintains related records and charts.
- Refer individuals to the appropriate services and resources.
- Performs all other duties as assigned.
Qualifications
- Lead therapeutic groups utilizing counseling techniques, theories, interventions, and ability to work one-on-one with patients daily.
- Ability to analyze, compare and evaluate aspects of the treatment program.
- Translate assessment information into measurable treatment goals and outcome statements.
- Maintain accurate and complete patient records and electronic files.
- Work with people in close situations.
- Understanding of HIPPA regulations and Clinical terminology.
- Prepare clear and accurate reports by using applicable software applications.
Education:
- Associate degree in psychology, sociology, social work, or a related field is required.
Experience:
- Five (5) years of experience working with consumers diagnosed with substance abuse, mental health, or dual diagnosis.
Licenses and Certifications:
- Valid Licensed Chemical Dependency Counselor (LCDC) license issued in Texas in good standing.
