Make a Difference
The Registered Nurse Navigator serves as a vital bridge between patients and the healthcare team, specializing in the timely identification of patient needs and coordination of comprehensive care services. This role combines clinical expertise with patient advocacy, focusing on educating patients about disease processes while monitoring and coordinating their care throughout the healthcare continuum. The RN Navigator for the Lung Nodule Program will initiate and maintain collaboration with appropriate healthcare team members to ensure prompt and accurate implementation of clinic protocols, establish therapeutic relationships with patients and caregivers, input orders for testing, coordinate care, attend weekly team meetings, patient education, monitor patients progress, and other related tasks as needed. The office is located on the north side of Indianapolis, Indiana and the nurse will work a hybrid schedule once training has been completed.
Exceptional Skills and Qualifications
-Graduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience. Bachelor of Science in Nursing (BSN) preferred.
-Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC).
-Two years or more of nursing experience required.
-Pulmonary/respiratory care experience strongly preferred.
-Experience in care coordination or patient navigation preferred.
-Excellent communication skills.

-EPIC experience a plus.
