Family Support Case Manager
- Salary Range: $24.04 - $28.85/hour based on relevant years of experience
- Location: Larimer county, Colorado-Orchards
- Status: full time, 40 hours per week, some evening work required
Role Overview:
The Family Support Case Manager provides case management services to families, including grandparents and other caregivers raising nonbiological children. Family Support programming offers support groups, parenting training, linkage to agency and community resources, advocacy for families with community systems, and coordination of social events. The ideal candidate for this position will work independently, be skilled at making connections with diverse groups, including clients and donors, and be passionate about supporting and advocating for families in our community.
Essential Duties:
- Provide ongoing case management to families, often focused on school enrollment, accessing state benefits, custody and other legal issues, accessing mental health and substance use treatment, and emergency needs.
- Respond to referrals quickly (within 48 hours, if possible). Help families complete a program-specific intake.
- Organize and distribute emergency materials for families: backpacks/school supplies, diapers/wipes, hygiene products, coats, etc.
- Provide information about upcoming events, support groups, and community resources.
- Provide information about emergency funding and gift cards.
- Facilitate monthly support groups for caregivers and their families.
- Identify additional training and educational needs for caregivers.
- Attend donor-sponsored events for caregivers and their families (typically fall and summer events).
- Maintain relationships with community partners, particularly Realities for Children, Office on Aging, and the Foothills Service League.
- Manage gift drives during the holiday season.
- Relevant bachelor’s degree (i.e. social work, human services, or related field)
- Applicants must be able to have an applicable degree to be on the CAS track. This would entail ongoing supervision and CAS trainings.
- 1-3 years minimum of directly relevant experience in the mental health & substance use fields
- Experience in coordinating services with other agencies
Preferred:
- CAS preferred
- Bi-lingual / Bi-cultural
This position will include driving a company vehicle and require a valid Colorado driver’s license
