Position Summary
The Patient Care Coordinator / Emergency Room (ER) Coordinator plays a vital role within the Population Health department by supporting smooth care transitions for patients seen in the emergency department or hospital. This position focuses on patient outreach, care coordination, follow-up scheduling, and accurate documentation within electronic health records (EHR) systems.
The coordinator contributes directly to improving quality metrics, patient outcomes, and continuity of care, while supporting value-based care initiatives through Aledade, CRISP, and eClinicalWorks (ECW). The schedule for this position is Monday - Friday 7:00 AM - 3:30 PM.
Key Responsibilities
- Monitor emergency department reports and admissions using Aledade.
- Conduct patient outreach after ER visits to assess well-being, discuss care plans, and schedule follow-up appointments with primary care providers (PCPs).
- Provide empathetic communication and education on the importance of follow-up care, medication refills, and ongoing health management.
- Retrieve and upload discharge summaries, labs, and imaging from CRISP into eClinicalWorks (ECW) under the appropriate categories.
- Document all patient interactions, outcomes, and next steps in ECW and Aledade accurately and timely.
- Update and maintain patient records, marking inactive, deceased, or transferred patients as needed.
- Schedule ER, hospital, and transitional care follow-up visits using correct visit types and color codes.
- Support quality performance goals related to transitional care management (TCM), ER follow-ups, and preventive health measures.
- Collaborate with clinical and administrative teams to promote care continuity and patient safety.
- Maintain strict HIPAA compliance and safeguard patient confidentiality.
- Complete CMS-required Transitional Care Management (TCM) calls within 48 hours of hospital discharge.
- Serve as backup to the Patient Informatics – Inpatient Coordinator when needed.
Qualifications
- Education: High school diploma or equivalent required; Associate degree in healthcare, public health, or a related field preferred.
- Experience: 1–2 years of experience in a medical office, population health, patient coordination, or care management setting.
- Technical Skills: Proficiency in Aledade, eClinicalWorks (ECW), and CRISP required.
- Strong knowledge of HIPAA, care coordination workflows, and patient confidentiality standards.
- Exceptional communication, organization, and multitasking abilities.
- Proven ability to work independently and collaboratively within a fast-paced, patient-focused environment.
