Cornerstone Family Healthcare is actively recruiting for a Program Supervisor to join our growing team in Newburgh.
RATE OF PAY/SALARY: $30.98 per hour.
WORK LOCATION(S): Newburgh, NY
STATUS: Full-Time
Schedule- Monday-Friday 9:00AM-5:00PM
JOB SUMMARY:
The Program Supervisor is responsible for direct service delivery and oversight of staff providing harm reduction services to PWID/PWUD including provision of referrals, addressing SDOH, Narcan training and communicating community trends needs within Sullivan and Orange Counties (SC/OC).
Key Competencies:
- Supervision of all aspects of the assigned programs to ensure they are meeting program deliverables and are in compliance with agency policies and funder requirements and standards. Supervise, train and develop staff.
- Follow through with staff on data entry, reporting and delivery of interventions and adherence to program standards.
- Continually review, develop and operationalize new strategies and systems so the program operates at maximum efficiency and is compliant with the most recent data trends and funder requirements.
- Manage and conduct outreach to community partners and maintain collaborative relationships with service providers to ensure referrals to the program and to more easily provide access to continuum of care for clients.
Description of Duties:
- Supervise staff, including Health Hub Harm Reduction Specialists (OC and SC), Program Assistant (Monticello office), and Prevention Specialist and Peer (OC).
- Provide training, treatment and testing updates, and other programmatic development to staff and peers.
- Conduct and oversee training sessions for the Opioid Overdose Prevention Program (Narcan).
- Compile program data, ensure that all required data is entered into the AIDS Institute Reporting System (AIRS), Cornerstone’s electronic medical record (ECW), and report preparation in accordance with funder’s requirements.
- Maintain ongoing relationships with administrators and staff at community-based organizations and local police stations.
- Continually conduct quality assurance reviews to ensure the maintenance of client charts, adherence to program standards, the goals and objectives of the workplan and compliance with agency policies.
- Assist in program coverage when needed.
- Provide HIV and HCV counseling on treatment and/or prevention, as appropriate, and provision of referrals to address SDOH.
- Ensure that services are trauma-informed, based on a harm reduction model that is culturally responsive, to produce positive outcomes for clients.
- Observe and abide by HIPAA and the HIV Confidentiality Law.
- Continually review and assess the demand for services in the communities where the programs are located and adjust and/or develop programming as needed.
- Encourage and maintain relationships with community-based resources helpful to the program’s priority population.
- Conduct outreach and recruitment activities using program presentations to service providers, via social media, and through networking with community organizations and key community members.
- Complete documentation, data entry and reporting in a timely, comprehensive manner.
- Attend and participate in trainings, department, All Staff and other required meetings.
- Be familiar with Cornerstone policies and procedures and the Employee Handbook.
- Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations.
- Perform other related duties as assigned.
Requirements
- Master’s degree in social work or related field;
- Alternatively, a Bachelor’s degree with experience in outreach, HIV and HCV counseling and prevention; and 2 years supervisory experience.
- Minimum one year experience in a leadership or supervisory position;
- Knowledge of HIV/HCV/STI, harm reduction, mental health and community resources;
- Trained in opioid overdose prevention including the use of injectable/intranasal naloxone.
