Clinical Social Worker, New Century Health Plan chez UCLA Health

Step into a vital role at New Century Health Plan and coordinate the care for our Medicare Advantage members. As a Clinical Social Worker, you will focus on addressing members’ complex social needs to help improve health outcomes and streamline their care journey. In this position, you will work closely with members, their families, community organizations and healthcare providers. Your responsibilities will include: Providing a range of social work services responsive to identified problems (e.g., psychosocial functioning, adaptation to illness, facilitating services, protective services, post-hospital planning, etc.) Maintaining assigned care management caseload with a focus on the most complex, highest-risk members particularly those with advanced chronic conditions, co-occurring mental and/or substance abuse and complex social issues (e.g. homelessness, domestic violence).  Assessing members’ healthcare needs through comprehensive psychosocial assessments. Collaborating with Nurse Care Managers to provide support to members enrolled in the Complex Care Management and Disease Management programs. Providing education and resources to members about accessing services, community resources, and self-management skills and strategies. Salary Range: $54.54-$65.18/hourly Postuler maintenant
