Position Summary
The Hospital Liaison for Long Term Care and Rehabilitation, plays a crucial role in facilitating admissions and enhancing collaboration between healthcare facilities and The Sanctuary of Holy Cross. As a representative, you will work closely with hospital discharge planners, case managers, and medical professionals to ensure a seamless transition for patients into our long-term care facility. This role requires strong interpersonal skills, an understanding of healthcare and sales, and the ability to engage effectively with hospital staff and patients' families.
Responsibilities
- Act as the primary liaison between hospitals, The Sanctuary of Holy Cross, and other healthcare providers to promote our facility and services.
- Cultivate and maintain positive relationships with hospital discharge planners, physicians, social workers, and healthcare professionals.
- Educate hospital staff, patients, and families on the benefits and services of The Sanctuary of Holy Cross.
- Coordinate patient admissions from hospitals, ensuring compliance with regulations and facility policies.
- Regularly conduct in-service presentations, tours, and informational sessions for hospital and community groups.
- Analyze and report on market trends, referral patterns, and feedback to optimize outreach strategies.
- Work collaboratively with clinical staff to develop personalized care plans for new admissions.
Preferred Qualifications
- Sales or Medical Sales Experience: Proven experience in sales, with a background in healthcare or medical sales preferred.
- Hospital Experience: Familiarity with hospital settings and patient discharge planning.
- Healthcare Knowledge: Understanding of the long-term care industry, insurance, and regulatory requirements is highly valued.
- Strong organizational, communication, and relationship-building skills.
