Outpatient Behavioral Health Care Coordinator
Part-time (20 hours per week)

General Responsibility:
The Outpatient Behavioral Health (OBH) Care Coordinator is primarily responsible to provide various outreach, support and screening services to assess and connect patients to their appropriate behavioral health needs. The OBH Care Coordinator is accessible and visibly present to OBH Clinicians and Primary Care teams during clinic hours.
Specific Responsibilities:
- Assisting with managing referrals for Outpatient Behavioral Health services through completion of phone assessments, referral triage and tracking.
- Managing scheduling for initial OBH appointments and missed appointments for clinical team.
- Outreaching patients via telephone to address immediate and urgent needs, determine need for further assessment or treatment either by the health center or by referral to another organization
- Assisting Outpatient Behavioral Health Clinicians and Director with tracking and outreach for patients needing follow-up appointments.
- Assessing and communicating any urgent clinical needs/crisis calls from patients to BH team for immediate assistance.
- Obtaining insurance authorizations as necessary.
- Participating in Behavioral Health staff meetings, multidisciplinary teams, and individual supervision sessions with the Outpatient Behavioral Health Director.
- Maintaining updated community resource and patient education materials related to BH (community health resources, insurance information, chronic disease management and social determinants of health).
- Participating in continuing education programs, including one (1) annual training program on compliance with OSHA's Blood borne Pathogen Standard.
Required Qualifications:
Bachelor’s Degree in social work, psychology or related field required. Previous work experience in a medical setting or client/patient centered community agency preferred. Experience serving a population from diverse educations, economic and cultural backgrounds. Experience with case management preferred. CPR Certification. Bilingual is an asset, but is not required.
