Are you passionate about integrating physical and behavioral health to improve lives? Join our dynamic team at the Certified Community Behavioral Health Clinic (CCBHC) as a Medical Case Manager. In this vital role, you’ll work closely with a multidisciplinary team to coordinate care, support transitions, and promote wellness for individuals navigating complex health needs.
Core Responsibilities
- Coordinate care across providers and settings (e.g., hospitals, skilled nursing, correctional facilities).
- Collaborate with internal and external partners to ensure seamless communication and care integration.
- Support day-to-day treatment coordination and preventative care services.
- Educate individuals on referred services and ensure satisfaction through proactive support.
- Assist with transitions following higher levels of care and support treatment plan outcomes.
- Maintain accurate documentation and manage health records and supplies.
- Participate in community events, trainings, and interdisciplinary meetings.
Qualifications
- Bachelor’s degree (preferred) or 3–5 years in a related medical field.
- Fluency in medical terminology.
- Valid NYS Driver’s License.
