- Grant: Family Spirit Program; 5 year grant

Family Spirit Home Visitor
Specific responsibilities may include, but are not limited to:
- Serve as a Family Health Educator for the Family Spirit project in Montezuma Creek
- Recruit young mothers to participate in the Family Spirit Program
- Visit young mothers in their homes and teach a comprehensive parenting curriculum
- Provide social support and connect young mothers to other community resources
- Maintain data forms and tracking logs to document study activities
- Represent the Family Spirit Program and the organization’s other programs during community meetings and in-service presentations for community partners and local service providers
- Travel to other sites for trainings, meetings and other duties as needed
- Assist with other organization activities as needed
- Service 15 Families with 85% goal of visits with lessons completed.
- Enter visiting data into Visit tracker home visiting database after every visit.

Requirements
Education and Experience
- High School Diploma or GED required. Bachelor’s degree in a health or social services field, preferred.
- Minimum of one (2) year of work, school or volunteer experience beyond high school required.
- Previosu experience working with young children and expectant mothers
- Community Health Worker/Community Health Representative perferred.

Family Health Educator -
- Good oral and written communication skills, including public speaking with large and small groups
- Ability to operate a telephone, printer, fax, copier, and computer with software use experience in Microsoft Word, Excel, and Power Point
- Ability to drive a project vehicle as needed and to follow guidelines on vehicle use
- Ability to follow worksite guidelines on personal and work cell phone use
- Ability to work well with others
- Ability to take direction and respond positively to feedback in the workplace
- Ability to maintain highest level of confidentiality
Other:
- Current driver’s license and good driving record (hired candidate will need to provide a copy of a current driver’s license in the state where the position is located)
- Ability to travel with overnight stays required as needed. Some evening and weekend work maybe required.
- This position will maintain current CPR, BLS or approved alternate certification.
- This position will maintain background clearance as required by the employer and any collaborating agencies.
As a Tribal Organization the Utah Navajo Health System, Inc. (UNHS) treats patients with high risk and underlying chronic medical conditions. Therefore, UNHS requires its employees to show proof of immunization prior to their employment with UNHS or during their employment. Below is a list of immunizations/vaccinations required to be employed or to continue employment with UNHS.
The immunizations/vaccinations include but not limited to following:
- MMR (Measles Rubeola, Mumps, Rubella): Documentation of two MMR vaccines (OR) Documentation of MMR titers.
- Complete Hepatitis B vaccine series (3 doses), documented proof of titers indicating immunity, or a declination that may be signed upon arrival.
- Proof of up-to-date PPD skin testing, if previously negative (OR) Proof of positive PPD skin test with the most recent chest x-ray and treatment history.
- Tetanus, diphtheria, and pertussis (Tdap).
- Varicella titers, history of varicella or varicella immunization series.
- Influenza vaccine for the current year.
- COVID-19 vaccine
- Any other vaccinations as requested.
