Description

General Summary:  A physical therapist (PT) must work as a clinical leader of the rehabilitation team to provide physical therapy services under the clinical direction and supervision of the Medical Director of Physical Medicine and Rehabilitation. PTs interpret prescriptions from physicians and/or non-physician providers, evaluate patients, and develop individualized plans of care and goals for patients based on findings from the evaluations. PTs also initiate treatments according to the plans of care, obtain data related to the interventions provided, make modifications in selected interventions either to progress the patient/client or to ensure patient/client safety and comfort, and document in a timely and accurate manner the treatments given to patients, including collecting frequent subjective data and objective measurements and correlating them to the established goals in the plan of care.  PTs also supervise, collaborate and communicate with, and hold regular meetings with PTAs to facilitate an environment for superior patient care and satisfy legal regulations. 
PTs support the company mission and goals by providing accurate and timely evaluations and physical therapy diagnoses with appropriate plans of care, while performing high quality evidence-based treatments and excellent customer service to individuals who have medical problems or other health-related conditions that limit their abilities to move and perform functional activities in their daily lives. Each treatment should be tailored to progress patients towards goals according to the plan of care.
    
Supervisory Responsibilities: Physical Therapist Assistant(s), Technician(s): clinical supervision (non-administrative), as needed for quality patient care.

General Requirements: All duties performed will be done accurately and in a timely manner. 

1.    Assumes responsibility for maintaining clinical competencies according to Gonzaba Medical Group policy.
2.    Ensures customer service is always maintained at the highest level. 
3.    Exercise tact and courtesy when dealing with patients, visitors, providers, and co-workers.
4.    Must always adhere to customer service expectations including in-person and virtual (via telephone, or telehealth applications) communication. 
5.    Participates in educational programs as needed.
6.    Other duties as assigned.


Essential Job Responsibilities:    
1.    Interprets physician/non-physician provider’s referral accurately and requests interpretation as necessary.
2.    Administers and interprets tests and measurements of patients for examination and evaluation purposes.
3.    Reviews patient chart and progress prior to treatment and modifies the treatment plan accordingly.
4.    Develops realistic functional goals based on patient and/or family expectations.
5.    Instructs and involves patient, family, PTA, social worker, nursing staff and discharge planner in patient’s progress and discharge plans, participating in appropriate meetings.
6.    Performs selected tests and measurements according to all regulatory agency requirements, within the Physical Therapy Scope of Practice and state and local laws.  
7.    Uses appropriate modalities and procedures correctly demonstrating knowledge of indications and contraindications.
8.    Performs therapeutic procedures by administering manual exercises; instructing, encouraging, and assisting patients in performing physical activities, such as non-manual exercises, ambulatory functional activities, and daily-living activities; and in using supportive devices, such as crutches, canes, and prostheses.
9.    Instructs and involves patients, family, and/or caregivers in patient’s progress and discharge plans making appropriate discharge recommendations to the therapist and attends appropriate meetings.
10.    Demonstrates working knowledge of and ability to follow and implement departmental policies and procedures.
11.    Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served, demonstrating knowledge of principles of growth and development over the lifespan.
12.    Completes all necessary documentation in a legible, appropriate, concise, and timely manner while assuring compliance.
13.    Assures appropriate and safe equipment performance prior to each use.
14.    Provides in-service to staff as needed or requested.
15.    Possesses working knowledge of and demonstrates adherence to departmental policies and procedures.
16.    Organizes schedule to reflect patient priorities and needs of other staff.
17.    Delegates responsibility to supportive personnel appropriately and assures appropriate supervision and oversight for safety and compliance.
18.    Provides clinical education/instruction for PT/PTA students as needed or requested.
19.    Informs supervisor of incidents, accidents, equipment failure or unusual physician or patient problems immediately    
20.    Utilizes resources efficiently and effectively.
21.    Maintains safe environment for patients and co-workers at all times.
22.    Addresses patient population-specific needs
23.    Maintains legal and accreditation compliance by adhering to policy positions concerning federal, state, and local regulations, and Joint Commission standards; anticipating emerging issues.
24.    Participates in Continuous Quality Improvement activities reporting by completing all requested CQI measurement and documentation in a timely manner. 
25.    Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
26.    Other job or business-unit related duties as assigned or requested.
 

Work Environment: Clinic environment. Exposure to communicable diseases, bodily fluids, toxic substances, ionizing radiation, medicinal preparations, and other conditions common to a clinic environment.
    
Mental / Physical Requirements: Requires manual dexterity, sitting, standing, stooping, reaching, kneeling, crouching, bending, walking, lifting up to 40 lbs. without assistance. Close vision and ability to adjust focus. Must be able to work efficiently under pressure.

Qualifications


Education and Training: Current PT licensure (TX) and BLS/CPR certification. CPR certification with AED is required and must be kept current. Master’s or Doctorate degree in physical therapy from an accredited PT program.

Experience: Clinical experience in outpatient physical therapy care, as well as marketing and business familiarity. No previous work experience needed.  Demonstrates computer knowledge of programs related to position and displays competency on an ongoing basis.  
    
Other Requirements: Computer Skills: Skilled in use of computer/EMR systems. Knowledge of Word processing software, spreadsheet software, Internet, and database software.
