Starting Pay Range –

$100,000+ annually depending on experience.

Summary:

This member of the Mobile Health team provides direct medical services; assessment and diagnosis; oversees standing orders; refers to specialty medical care and other community providers. This position entails working on a mobile unit in the community as part of a multi-disciplinary team providing care to houseless and unstably housed individuals. 

Essential Job Responsibilities:

  • Coordinate and implement activities of the Medical Department to meet consumer needs, contractual obligations and agency standards.
  • Works in an independent and interdependent relationship with members of the medical staff and other providers, including consultation, collaboration or referral.
  • Prescribes medications, treatments and ongoing individualized medication management.
  • Maintains and completes legible, accurate and confidential medical records within 24 hours of patient encounter
  • Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations according to Agency, MQAC and Federal standards.
  • Coordination of care with other providers, counselors and community care providers.
  • Coordinates mental health care needs with their medical/physical care as needed.
  • Orders lab and diagnostic tests when appropriate and interprets rest results. Oversees standing orders.
  • Triage acute and chronic medical conditions. Using appropriate judgement to determine which conditions can be treated by this team and which should be escalated to urgent or emergency care
  • Under the direction of the team manager and director, organize and manage the system of providing medical outreach and integrating medical services tightly into clients’ individual treatment plans.
  • Coordinate, schedule, and administer medical assessments of client physical health, making appropriate referrals to team prescriber for further assessment and treatment
  • Provide assessment and appropriate treatment to clients with substance use disorders. Including COWS assessments, impairment assessments and emergency interventions for over-sedation.                                                                                                 

Non-essential Job Functions:

  • Support agency mission, goals, and objectives.
  • Attend staff and planning meetings as scheduled. 
  • Maintain communication and cooperative working relationships both within the agency and community at large.
  • Active driver’s license and valid insurance required.
  • Other duties as assigned by Supervisor.

Education, Licensing and Experience:

  • An active, unrestricted Physician, Advance Registered Nurse Practitioner or Physician Assistant License
  • CPR certified
  • One year or more experience in behavioral health prescribing preferred
  • Experience working with the aged, the uninsured, the underinsured, individuals with severe mental health disorders, and/or substance use disorders preferred.
  • Ability to credential with Medicare, Medicaid and commercial insurers
