We are looking for a Licensed Practical Nurse to work in our Community Health Center in Southeastern Utah. Our clinic is a dynamic place to work, practice, and grow. We have 4 primary care health centers and deliver integrated services including behavioral health, Pharmacy, Dental, specialty referrals, chronic disease management, health education, and much, much more.
  • Location: Blanding, Utah

Critical Tasks:

  1. Reduce the risk of infections
  2. Educate patient regarding their healthcare and the healthcare system for patient/family to effectively care for patient
  3. Perform nursing assessment and identify pertinent information to verbalize to provider.
  4. Review and implement primary care provider orders,
  5. Safely administer medications
  6. Perform nursing evaluation and follow-up with patient for continued comprehensive care
  7. Participate in ongoing education and training
  8. Assign and delegate tasks appropriately when necessary and accept assignments and delegated tasks when necessary according to Utah Nurse Practice Act

Requirements

Key Performance Indicators:

  1. Maintain proper hand-hygiene
  2. Use personal protective equipment (PPE) according to Center for Disease Control & Prevention (CDC) recommendations
  3. Perform low-level disinfection of medical equipment, devices and supplies
  4. Properly sterilize equipment and supplies.
  5. Maintain supplies, including stocking and ordering new supplies
  6. Properly dispose of expired or damaged supplies, medical equipment and devices.
  7. Remove all expired medication/injectable and products in crash cart and place order with administrator or pharmacist for replacement
  8. Assist patient with scheduling appointments
  9. Follow up with patients who miss appointments.
  10. Address patient telephone inquiries by evaluating and responding to the patient's problems and questions.
  11. Educate patient about healthcare system, what to expect, how to prepare, and be self-empowered
  12. Confirm medical records are available in patient chart before follow-up appointments
  13. Screen patient for depression, changes in tobacco use, caffeine use, diet, health literacy level, preferred method of learning, and medication allergies.
  14. Obtain patient's vital signs, pain level and weight.
  15. Educate patients about immunizations verbally, and give written Vaccine Information Sheet (VIS)
  16. Collects samples, such as urine, blood, and sputum, from patients for testing and follow-up needs with patients.
  17. Interview patient to determine medical problem/condition and document in chart for provider.
  18. Assemble and use oxygen supplies
  19. Order diagnostic procedures and labs as directed by primary care provider with verbal order read back
  20. Write prescriptions at providers request with verbal read-back of order
  21. Update patients on their immunization schedule according to CDC recommendations
  22. Dress wounds, apply hot or cold compresses as ordered.
  23. Educate patient about medication side-effects, disease, comorbidities, family planning and wellness
  24. Administer medications as prescribed, orally, subcutaneous, or intramuscular injection, and document time and amount in patient’s chart.
  25. Observe patient(s) and report adverse reactions to medication or treatment to the provider.
  26. Follow up with patients on lab results, including the scheduling of any additional tests.
  27. Write referrals with verbal read back of order for patients through insurance and other physician offices and follow-up.
  28. Attend mandatory monthly meetings
  29. Locate and attend meetings, online webinars, conferences and training that emphasize your role/responsibilities
  30. Locate articles and stay up to date on latest evidence based practice regarding your role/responsibilities
  31. Complete yearly competencies as assigned by supervisor
  32. Ensure that a delegated task you delegate is non-complex and personnel is adequately trained for the task before delegating the task
  33. Ensure that personnel has effectively demonstrated an assigned task as learned
  34. Ensure that personnel can perform an assigned task safely and patient is safe for personnel to carry out the task
  35. Ensure appropriate supervision is available during implementation of an assigned task
  36. Evaluate effectiveness of nursing plan of care from assignment and modify plan as needed when assigning a task
  37. Assign to other Licensed Practical Nurse (LPNs)
  38. Delegate as necessary to unlicensed assistive personnel [UAP (including Medical Assistants)] and Certified Nursing Assistants (CNAs) after Registered Nurse (RN) assessment and consistent with plan of care.

Metrics:

  1. Document patient assessment in patient’s health record prior to patient scheduled appointment or walk-in time slot with provider. Document on the note under Vital Signs in Athena to identify changes or abnormal vital signs if provider or patient’s nurse is unavailable.
  2. Document patient orders with verbal order read back (VORB) to provider or standing orders (SO) within a 24 hour time frame of patient’s visit.
  3. Document nursing interventions, evaluations, and notification to patient provider to patient’s health record within a 24 hour time frame of patient’s visit.
  4. Document patient/family and healthcare team communication regarding appointments, labs, referrals, and medical records, including faxed/requested items, in patient’s health record within a 24 hour time frame of patient/family and healthcare team communication.
  5. Document assigned room cleaning and expired supply removal and return document to your supervisor on a monthly basis
  6. Schedule and hold monthly or quarterly rounding meeting with your direct supervisor

Experience, Training, and Qualifications needed to perform the job:

  • One year certificate from an accredited college or university
  • Licensed Practical Nurse certification within state of Utah
  • Must be able to safely lift, push and pull fifty (50) pounds or more above waist
  • Two years of clinical nursing experience.

License/Certification:

  • Must possess a valid driver’s license
  • Must successfully pass a criminal background check
  • Must obtain and maintain First Aid/Cardiopulmonary resuscitation (CPR) certification
  • Must receive annual inoculation of flu vaccine
  • Must receive annual tuberculin skin test

In accordance with Navajo Nation and federal law, UNHS has implemented an Affirmative Actin Plan pursuant to the Navajo Preference in Employment Act. The NPEA has the following preferences:

1) Enrolled Navajo (must have a CIB).

2) Applicant married to an enrolled Navajo and who has lived on the NN for more than a year.

3) Everyone else (all other tribes and non-Indians).

Therefore, UNHS requires its employees to show proof of immunization prior to their employment with UNHS or during their employment. Below is a list of immunizations/vaccinations required to be employed or to continue employment with UNHS.
The immunizations/vaccinations include but not limited to following:

  • MMR (Measles Rubeola, Mumps, Rubella): Documentation of two MMR vaccines (OR) Documentation of MMR titers.
  • Complete Hepatitis B vaccine series (3 doses), documented proof of titers indicating immunity, or a declination that may be signed upon arrival.
  • Proof of up-to-date PPD skin testing, if previously negative (OR) Proof of positive PPD skin test with the most recent chest x-ray and treatment history.
  • Tetanus, diphtheria, and pertussis (Tdap).
  • Varicella titers, history of varicella or varicella immunization series.
  • Influenza vaccine for the current year.
  • Any other vaccinations as requested.
