Summary:

The Senior Director - Population Health will report to the VP of Population Health. They will collaborate with value-based care (VBC) and Population Health leadership to assist with guiding strategic direction, design, implementation, and support of a robust enterprise-wide population health management organization. They will oversee value based program administration and provide vision and leadership aligning clinically integrated partners throughout the care continuum. The position is directly responsible for Medicare value-based payment programs such as Bundle Payments for Care Improvement (BPCI Advanced), advanced the technology enablement including advanced VBC analytics, and the post-acute provider network. The Senior Director - Population Health will work closely with the Director VBC Analytics, VP/Chief Quality and Patient Safety Officer, hospital leadership, and the MHN and Population Health Medical Director to ensure clinical and technology excellence and sharing of best practices system-wide.

Responsibilities:

Value-Based Care Programs: Collaborates with external and internal stakeholders to develop and support value-based programs and plans designed to enhance the health of target populations, quality of care, and patient safety while fulfilling contractual obligations. Remains up to date with the current healthcare landscape and develops associated programs and requirements to address the healthcare needs on both a local and national level. Works closely with the VP/Chief Quality & Patient Safety Officer, MHN and Population Health Medical Director and other MHS Medical Staff leadership to measure, evaluate, and manage value-based care and aligns programs with payer contracts through quality and pay for performance metrics. Works collaboratively with the Medical Staff Leadership and the CNOs to implement clinical transformation processes to ensure effective implementation and use of chronic condition specific order sets, care plans, and care pathways. Oversees and directs the Value-Based Care Program Managers and the development of system task forces for care transformation and SNF Navigation for post-acute management of patients. In addition, oversees the operations and contractual agreements in the assigned area of responsibility to ensure they relate to strategy, execution, adherence and maintenance of effective care and network management. Provide feedback and make recommendations on care management progress and technology advancements to maximize contractual incentives, ensure alignment of goals and streamline performance improvement initiatives. Responsible for program development that effectively evaluates assures health equity and addressing social determinants of health and provides infrastructure for community service referrals for at-risk patients.Post-Acute Care (PAC) Network: Responsible for the design, development and implementation of a post-acute preferred provider network. Will work collaboratively with the post-acute Executive Leadership, Physicians, the Home Health and Skilled Nursing Facility leadership to develop a framework to manage the post-acute provider network. Responsible for driving change through a strong PAC partnership through leadership of the PAC Alliance. Manage PAC agreements and charters to ensure effective management of all VBC lives in achieving target quality and cost goals. Develops and maintains relationships with PAC leadership to lead by example and create an environment that fosters a PAC Network of excellence. Assists with the design, development and implementation of clinical transformation initiatives to drive safe transitions of care through effective communication tools, training of PAC staff, use of care plans and care pathways.Population Health Analytics: Defines and develops analytic and population health strategies that align with organizational strategic priorities. Implements data driven programs that assist in the identification and risk stratification of high-risk populations with the goal of improving quality and outcomes, while reducing costs. Develops analytics and programs to address care gaps in patients while also evaluating the socioeconomic needs of patients. Provides value based partner framework to manage Provider performance metrics, cost and utilization and other custom reporting needs. Works with IT clinical systems to develop inline dashboards and other analytic tools while continuously re-evaluating programs, reporting tools, and workflows to stay ahead of the market in VBC analytics.Population Health Management: directs, oversees, and collaborates with the Director of Population Health Management in the development, improvement, and management of the comprehensive care management program of Memorial ensuring the highest level of integrated care coordination and cost-effective quality care. Serves as a clinical advisor for care management across the healthcare system. Provides leadership and education on the importance of social determinants of health. Contributes towards decisions regarding alternatives to acute care in consideration of quality of care and financial factors. Develops and maintains relationships with all departments (clinical and non-clinical) within Memorial and develops relationships with post-acute providers to streamline the patient's transition of care and navigation through the healthcare system to the community setting post discharge. Must be comfortable operating in a collaborative, shared leadership environment.
