SPECIFIC RESPONSIBILITIES

· Leadership and Administration:

o Provide overall leadership and management for care management services for adults and children, ensuring services are aligned with NYS Health Home and other relevant state program standards.

o Develop and implement operational policies and procedures to ensure regulatory compliance and high-quality care.

o Supervise, mentor, and evaluate care management supervisors and staff, fostering a culture of continuous improvement and professional development.

o Oversee enrollment, assessment, care planning, service coordination, and discharge processes for members, ensuring all are completed per NYS requirements.

o Ensure timely and accurate documentation in accordance with state and federal regulations.

o Serve as a liaison with state agencies, community organizations, and healthcare providers to promote integrated care and address service gaps.

o Maintain up-to-date knowledge of NYS regulations, Health Home standards, and best practices for care management.

o Ensure all staff complete required NYS Health Home and care management training.

o Assist in audits, surveys, and reporting as required by NYS and other regulatory bodies.

· Fiscal and Resource Management:

o Assist with managing program budgets, ensuring efficient use of resources and staying within financial constraints.

o Monitor and manage performance metrics, including service delivery targets, utilization, and outcomes.

o Ensure appropriate documentation and billing practices are in place for reimbursement through Medicaid, Medicaid Managed Care, and/or other applicable systems.

III. QUALIFICATIONS

· Education and Experience:

o Master’s degree in Social Work, Nursing, Public Health, Healthcare Administration, or a related field required.

o Current NYS licensure as an LMSW, LCSW, RN, or equivalent preferred.

o Minimum of five (5) years of progressive experience in care management, case management, or care coordination, with at least two (2) years in a supervisory or management role.

o Demonstrated experience working with both adult and pediatric populations, including those with chronic conditions, behavioral health needs, and/or complex social needs.

o Strong knowledge of NYS Health Home program standards, Medicaid Managed Care, and relevant state and federal regulations.

o Excellent leadership, organizational, and communication skills.

o Ability to analyze data, monitor outcomes, and implement quality improvement initiatives.

o Proficiency with electronic health records and care management software.

· Skills and Competencies:

o Knowledge of NYS care management regulations for both adults and children, including Health Home standards.

o Ability to lead multidisciplinary teams and manage complex program operations.

o Strong interpersonal, advocacy, and problem-solving skills.

o Strong leadership and team-building skills with the ability to manage and motivate multidisciplinary teams.

This is a hybrid position with travel between our Brooklyn and Queens offices. Occasional travel to our Suffolk and Nassau County offices for meetings, training and program coverage. This position requires flexible hours to meet program and regulatory needs.
