Description
JOB PURPOSE: The home health aide is responsible for providing in-home personal care and
other services to patients in their home.
QUALIFICATIONS:
- CNA from the Maryland Board of Nursing
- High school diploma or GED.
- One-year experience preferred.
- Successful completion of written examination with a score of 70% or higher.
RESPONSIBILITIES:
- Perform complete personal care services to patient as assigned by the RN.
- Measure and record vital signs - temperature, blood pressure, pulse, respirations.
- Transfer patients in and out of bed using proper body mechanics and/or equipment.
- Assist patients with ambulation.
- Perform range of motion exercises as instructed and supervised by SN/Therapist.
- Perform light housekeeping and meal prep duties as assigned.
- Document and submit daily visits according to Agency guidelines.
- Report changes in patient condition promptly to RN or Clinical Supervisor.
- Call nurse case manager weekly with patient review for past week.
- Communication skills in regard to the aide’s ability to read, write, and verbally report clinical information to patients, representatives, and caregivers, as well as to other HHA staff.
- Recognize and reporting changes in skin condition.
- Perform patient exercises as assigned and supervised by therapist.
- Attend staff meetings and in-services as required.
- You have the ability and opportunity to complete additional tasks in the office if and when your assigned patient schedule does not afford you the total number of working hours in a week that your position allows. Additional office tasks include but are not limited to: filing, copying; assistance in medical records and completing admission packets. If you choose not to perform the tasks offered you have the ability to use approved PTO or leave without pay, pending supervisor approval.

SPECIAL REQUIREMENTS
- The HHA will work one weekend per month.
- Possess and maintain documentation of freedom of TB in a communicable form.
- Possess and maintain documentation of annual influenza vaccination.
- Complete annual mandatory competencies as defined by the agency Education Plan
- Maintain current CPR and First Aide certification.
- Maintain current driver’s license.
- Maintain current auto insurance of at least $100,000 total liability insurance per person and $200,000 total liability insurance per accident.

PHYSICAL DEMANDS:
The physical demands described here are representative of those that must be met by an
employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling and stooping all day. The employee must frequently lift or move items weighing over 20 pounds. This position requires the ability to transfer, lift, move, and turn patients to and from bed, wheelchair, toilet, tub, shower, and occasionally the floor.
WORKING CONDITIONS:
- Work is primarily performed in the patient’s home.
- Exposure Risk to Blood borne Pathogens: Category A - routine exposure to blood and body fluids. Occasional exposure to hazardous materials and infectious diseases.
- Extensive travel is required, sometimes in hazardous weather conditions.
EMERGENCY RESPONSIBILITY:
Expected to respond to emergency situations involving the safety of the patient in their homes. This includes the ability to assist with a possible evacuation of patient, or notify the proper agency/facility in case of any type of emergency situations such as calling an ambulance or the fire department.
