RN Intake Evaluator | Kent County Senior Millage (KCSM)

Part-time | 3 days per week | Kent County

Reliance Community Care Partners (RCCP) is seeking an RN Intake Evaluator for the Kent Count Senior Millage program.  This RN will determine program eligibility, schedule visits and perform in-person participant assessments.  This key team member is also responsible for planning, implementing and coordinating care plans to facilitate appropriate services across the continuum of care, providing referral management as needed to assist individuals with service program options.

Job Qualifications:

• Current, unrestricted nursing license in the state of Michigan
• Minimum of two (2) years of direct clinical experience
• Preferred experience for this position: home care, long term care, and case management
