SUMMARY:

 The Pharmacist in an Extensivist Practice plays a vital role in supporting patients with complex medical needs, particularly during transitions of care. This role focuses on medication optimization, reconciliation, and adherence strategies to reduce hospital readmissions, improve outcomes, and enhance patient safety. The pharmacist works closely with extensivist providers, care coordinators, and social workers to deliver high-touch, patient-centered care.

 PREFERRED QUALIFICATIONS:

  • Doctor of Pharmacy (PharmD) degree required.
  • Active pharmacist license in good standing.
  • Experience in ambulatory care, transitions of care, or population health preferred.
  • Strong clinical knowledge of chronic disease pharmacotherapy (e.g., diabetes, heart failure, COPD).
  • Excellent communication and patient engagement skills.
  • Familiarity with EMR systems and healthcare analytics tools.

PREFERRED SKILLS:

  • Board Certification in Ambulatory Care Pharmacy (BCACP) or Geriatric Pharmacy (BCGP).
  • Experience working in value-based care models or extensivist programs.
  • Knowledge of social determinants of health and their impact on medication access and adherence.
  • Ability to work independently and collaboratively in a fast-paced, patient-centered environment.

 PHYSICAL GUIDELINES:

 Physical guidelines include the ability to move, traverse, position self, remain in a stationary position and negotiate steps for up to eight hours per day.
