About the role:
The Advanced Illness Care Nurse is responsible for assessing and addressing the needs of patients and their families. This includes planning, implementing, evaluating, and documenting individualized palliative nursing care in alignment with the interdisciplinary care plan and Clinical Services Department policies. This role requires effective coordination and collaboration with the Interdisciplinary Group (IDG) and other healthcare providers to ensure comprehensive, high-quality care. Patients may be located in a variety of settings, including private homes, skilled nursing facilities, assisted living communities, or hospitals. The Advanced Illness Care Nurse consistently demonstrates excellence in clinical practice across their caseload and actively supports the mission of Bluegrass Care Navigators by delivering compassionate, patient-centered care to all individuals served.
Requirements
- A graduate of an accredited school for Registered Nurses.
- Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or participating COMPACT state Board of Nursing.
- One year of nursing experience required. Knowledge of palliative care or previous palliative care experience preferred.
- Skilled in nursing practice and able to maintain professionalism in moments of emotional stress.
- Knowledge of the process of symptom management, advanced directives, and death and dying.
- Has excellent observational skills, good nursing judgement, and communication skills. Ability to provide accurate and timely documentation.
- Must have strong organizational and planning skills and possess the ability to manage and interpret data.
- Is self-directed with the ability to work with little supervision.
- Experienced with coordinating clinical staff and providing effective education and feedback.
- Complies with accepted professional standards and principles.
- Understands hospice and palliative philosophy and principles.
- Possesses and maintains current CPR certification.
- Demonstrated ability to work well with health care professionals and an interdisciplinary team (IDT).
- Has the ability and expertise to manage multiple tasks and projects at one time.
- Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
- Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.
