- Make an Impact! The Evening and Weekend Care Team Leader is responsible for the coordination of client care activities among home health disciplines on weekends and evenings. The Evening and Weekend Care Team Leader ensures ongoing communication with providers to assure appropriate management of the client’s care

Position: Weekend Care Team Leader
Shift: Days; 2 out of 4 weekends; Hours are from 8:00 - 5:00 pm, possible 1 Friday evening per month from 5:00 - 9:00 pm
Experience:
3-5 year(s) of recent Home Care Nurse experience.
Education:
1. RN Diploma, ASN required.
2. BSN preferred.
Licensures/Certifications:
1. PA RN license.
2. Provider BLS (American Heart Assoc/American Red Cross) required.
Requirements:
This position requires you to wear a respirator as part of the job requirement.
