QUALIFICATIONS AND EXPERIENCE
1. Is a graduate of an accredited school of professional (registered) nursing. BSN preferred.
2. Maintains license as a Registered Nurse from the State of Michigan.
3. At least one (1) year's general nursing experience is required with current Hospice; medical, surgical, or critical care experience. Experienced in providing pain/symptom management, IV skills preferred.
4. Experience with issues concerning the terminally ill, and death/dying, and pain and symptom management.
5. Presents a negative TB skin test and/or chest X-ray and other tests as required by Centrica Care Navigators.
6. Maintains a valid Michigan driver’s license and has the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.
7. Remains up to date on routine immunizations requires by LARA and agency policy
SUMMARY OF JOB RESPONSIBILITIES
Admission Coordinator will convey Centrica Care Navigators services provided to prospective patients, families, and referral sources. The Admission Coordinator navigates the coordinator of scheduling the admission to ensure appropriate admission coverage in the community and Centrica Rose Arbor Place. The Admission Coordinator is responsible for applying the rules and regulations of the state and federal regulatory agencies. The Admission Coordinator is responsible for promoting a positive, therapeutic, and professional working relationship within the agency among all departments and all external stakeholders.
RESPONSIBILITIES AND DUTIES
Primary Duties
- Answers telephones and responds to general inquiry questions about our services.
- Obtains information and history from referral source. Triages patient needs and schedules informational and initiation visits appropriately.
- This role has oversight of hospice-to-hospice transfers to Centrica Care Navigators, ensuring compliance is maintained.
- Provides support with EMR for admissions including entering coordination notes, approval of DME, and ICD 10 diagnoses in a timely fashion.
- Assists with mentoring and oversight of start of care RNs with non-admits and assisting with hospice eligibility.
- Mentors and provides oversight of start of care RNs with non-admits and assisting with hospice eligibility.
- Maintains the follow up program by making or delegating phone calls. Documents follow up program interactions in electronic medical record and schedules visits appropriately.
- Provides support for the hospital liaisons both in the field and in the office.
- As requested by the Director, will assist with supervisory visits for the Start of Care Nurses.
- Coordinates and provides information to nursing homes, adult foster care homes, assisted living centers, and other facilities as needed.
- Processes workflow in electronic medical record as needed.
- Stays informed about changes in the field of Hospice care; shares information with appropriate Hospice personnel and administrative personnel.
- Promotes Hospice referrals in the community; maintains a good standing in the community.
- Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
