About the Space:
 

Since 2007, SFCHC’s Trans:Thrive has provided comprehensive medical and social services for the trans/GNC community of San Francisco. More than a drop-in center, Trans:Thrive is a safe space and “one stop shop” for community members to access programming to improve their quality of life - offering case management, medical services (including primary care, dental care, behavioral health services, gender affirming hormone therapy and surgeries), substance use services and treatment (including harm reduction supplies, counseling, treatment linkage), support groups, hot meals, and community events.

 

Programs:

  • San Francisco Transgender Alliance for Health Resources (STAHR): STAHR is an innovative, collaborative, client-centered Health Access Point (HAP) program specifically focused on addressing the needs of trans women in San Francisco (SF). STAHR operates through a close working partnership that brings together five highly respected SF agencies and programs, all of whom have distinguished and extensive experience in reaching, serving, and retaining trans women in HIV, HCV, and STD testing, care, and support. STAHR strives to reach and serve all trans women living in SF, with a strong emphasis on African American and Latina trans women.
  • House of Thrive (HOT): The HOT program is an innovative, multi-level intervention designed to build life skills, increase education and employability, and build healthier, more self-directive futures for low-income and marginalized trans people in SF. The H.O.T. Program is a year-long goal-setting case management program that supports a cohort of TGNC participants and is funded through the Mayor's Office of Housing & Community Development (MOHCD). 

Role Summary: 

The Case Manager will work closely with the Program Manager of Trans Services, the Program Specialist and Outreach and Community Engagement Specialist, and will be responsible for overseeing case management of clients in the STAHR and HOT programs. 

                  

Essential Duties:

  • Manage case management services of both STAHR and HOT programs by conducting intake, assessment, care plans, and re-assessment of eligible transgender clients
  • Provide case management services to ongoing caseload and develop individual-client centered goals, resource and care navigation, harm and risk reduction counseling, and treatment plans following agency’s standards of practice.
  • Assess and enroll eligible clients into Enhanced Care Management (ECM) to ensure smooth and consistent care coordination across SFCHC departments as well as other community clinics and health care providers.
  • Provide comprehensive linkage support, education, shelter, and treatment regarding PrEP, PEP DoxyPEP, HIV/STI testing, substance use, harm reduction, access to basic need services, primary medical care, and behavioral health services.
  • Work occasional community pop-up testing events on weekends for outreach.
  • Meet with clients on a regular basis to ensure client retention in medical care, behavioral health services, substance use treatment and prevention, stable housing and shelter, and other individual client goals, depending on acuity level.
  • Ensure timely and accurate notes and documentation are input in applicable systems such as EPIC, CareLink, GMS, ONE System, and other secure tracking systems.
  • Enroll clients in various medical care services including primary care, mental health services, needle exchange and syringe access, gender affirming care, and safer sex and substance use.
  • Provide supportive services and resources to trans clients at Trans:Thrive, other SFCHC sites, as well as inside various SF jails, psychiatric facilities, and rehabilitation sites.
  • Conduct outreach and recruitment activities to deliver peer-based HIV prevention services in venues where transgender communities congregate and socialize including city-wide events, clubs, bars, support groups, etc.
  • Perform quality data collection, data entry, maintenance, tracking of client enrollment/ activities, and timely submission of units of services for monthly, quarterly, and yearly reporting requirements. 
  • Promote and represent agency services at outreach events. 
  • Participate in trainings, monthly staff, program, and department meetings, agency events, and necessary HAP meetings for SFDPH as well as MOHCD grantee meetings.
  • Other duties and responsibilities as assigned.

Minimum Qualifications

 

  • Ability to sit, stand and walk at least 50% of the work hours (this position requires outreach). 
  • Two years of college or equivalent combination of education and experience.
  • Three years of experience working or closely associating with transgender individuals and issues.
  • Familiarity with issues and concerns of transgender sex workers and substance users; experience working with multiply diagnosed patients (HIV, mental health conditions, and substance abuse), sexual and gender minorities, people experiencing homelessness, immigrants and refugees. 
  • Experience working professionally and ethically within multi-racial and multi-cultural settings.
  • Ability to work independently as well as within a multidisciplinary team.
  • Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills in English
  • Detail-oriented with strong follow-through skills, including attention to accuracy and deadlines.
  • Available to work weekends, evenings, and non-traditional work hours.
  • Up to date with vaccinations, especially the latest COVID-19 and seasonal flu vaccine.
  • Proof of TB clearance (issued within the past 12 months).

 

Preferred Qualifications

  • Bilingual English and Spanish.
  • HIV/HCV/STI Test Counselling certification.
