Description
We are currently looking for a Licensed Practical Nurse to assist our personal choice and Residential Program. We are looking for an LPN who's available 2 or 3 days out of the week.

Essential Duties and Responsibilities:
- To provide competent professional medical services to all the individuals receiving day and residential services.
- To comprehensively deliver care to individuals utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation.
- To interact with individuals and their families, clinicians, and direct support staff effectively and professionally.
- Answer phone calls and document information as necessary.
- Call providers as needed for questions, or concerns, or obtain updates as needed.
- Notify clients families of any incidents.
- Visit clients in the group homes as needed.
- Attend staff meetings as needed.
- Coordinate services with the Physical Therapist, Occupational Therapist, and Speech as needed.
- Ensure bills are paid accurately and on time.
- On an annual basis, evaluate the all individuals’ medical conditions and its effect their daily functioning.
- Assist individuals in identifying and accessing available community resources in the areas of wellness and health promotion and/or maintenance.
- Report any health and safety concerns to the State Medicaid office and either the Division of Elderly Affairs (DEA), Protective Services, or the Department of Behavioral Health, and Developmental Disabilities Office of Quality Assurance (BHDDH) if exploitation, abuse, or neglect criteria are evident.
- Serve as a liaison between AccessPointRI and The Executive Office of Health and Human Services or other agencies which provide support for an individual in conjunction with the Residential Director.
- Perform all duties as needed.
Qualifications
Qualifications

Current unrestricted Rhode Island LPN license.

Minimum 2 years of nursing experience; experience in community health, home care, or long-term care preferred.

Strong assessment and documentation skills.

Ability to manage a large caseload with efficiency and attention to detail.

Knowledge of Medicaid/Medicare and self-directed service programs preferred.

Excellent communication and interpersonal skills with individuals, families, and interdisciplinary teams.

Valid driver’s license and reliable transportation for statewide travel.
