Position Purpose
New Foundations is a licensed community residential rehabilitation program serving eligible young adults between the ages of 18-22 requiring behavioral health support. Residents may also have a forensic history.
Psychiatric Rehabilitation Workers (PRW) will work under the direction of the Psych Rehab Specialist to assist participants in choosing, acquiring, and maintaining the skills they need to successfully transition using skills-based and strength-based techniques. PRWs will collaborate with program therapists, the Psych Rehab Facilitator, and residents to assist with the implementation of the Psych Rehab Plan of care to increase client skill levels through group facilitation and individualized psych rehab activities.
Major Responsibilities
- Promote principles of recovery and psychiatric rehabilitation with program residents and staff.
- Provide skill-building services including, but not limited to, assisting in recovery management, identifying personal strengths/assets, developing a strength-based recovery plan and goals, and creating strategies to meet goals.
- Utilize motivational interviewing to effectively identify the member’s strengths, needs, motivations, triggers, and goals in managing life circumstances.
- Participate in outings with residents to practice rehabilitative skills.
- Facilitate daily group and individual psych rehab activities for program residents as directed by PRS.
- Document daily activities in the electronic health record.
- Complete all other assigned tasks as designated by program leadership.
SCHEDULE: WEDNESDAY AND SATURDAY 3pm-11pm and THURSDAY AND FRIDAY 11am-11pm
Job Qualifications
- Valid driver’s License
- Pre-employment physical, and TB Test
Education and Experience:
- High school diploma or GED minimum; bachelor’s degree preferred.
- Minimum of six months of direct care experience with clients with mental health, substance abuse, or forensic issues.

Physical requirements
The Psych Rehab Worker will need to be able to be mobile to travel to participant apartments and have physical capability to do this. Walking and the ability to carry a laptop or other mobile devices, books, binders or other work items may be needed while traveling in the community.
Lifting Requirements
Ö Sedentary: exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to left, carry, push, pull or move objects (including self). Most work involves sitting majority of the time. |
Light: exerting up to 20 pounds of force frequently, and/or negligible amount of force constantly to move objects. The use of arm and/or leg control requires force greater than sedentary, but worker still sits majority of time. |
