The supervisory social worker works closely, in collaboration, with the PACT SW Program managers, to seamlessly manage the PACT SW team across 13 sites of care. The Supervisory Social Worker will be assigned staff to supervisor, across up to 3 sites of care, this includes training and mentoring assigned staff. The incumbent: Knowledge and ability to provide clinical supervision, consultation, and work across service lines and departments. Skillful communication and ability to navigate interpersonal relationships and dealings with employees, team leaders, and managers. Identifies areas for improvement, gather relevant data, assess the data, develop and implement ideas for improvement, and evaluate efficacy of improvements. Will participate in program development and other special duties and projects as assigned by the Chief Provides supervisory leadership to the PACT Social Work Team. Is an expert concerning the provision of case management services. Will assist less experienced and less knowledgeable staff with complicated medical and mental health conditions. Provides assistance for social work and other staff as needed or requested. This includes the interpretation of VHA Directives, Medical Center Policies, Program Handbooks, and applicable state laws and guidelines. Must possess and maintain a valid state driver's license and driving requirement of approximately 50%. Regularly address the most difficult, challenging, complex, and acute patient care needs encountered in the Medical Center treatment areas and communities served, including special populations. Coordinates and collaborates with the facility level case management teams to include nurse and social worker case managers. Establishes collaborative relationships with physicians, psychologists, pharmacists, and other disciplines to foster their involvement in the case management of Veterans. Advocates on behalf of case management services and facilitate communication with facility executive leadership on case management strengths and challenges. Collaborates with internal stakeholders to assess case management composition, structure and services to identify promising practices and gaps in case management services. Identifies opportunities to enhance communication, collaboration and coordination across the care continuum. Education, Coaching, Consulting and Mentoring: Directs activities to maximize effectiveness, efficiency and continuity of care for veterans. Provides case management services to those veterans at high social, psychiatric, or medical risk, serving as the liaison to other treatment programs. Represents the program in contacts with other agencies and the public. Provides orientation and on-going training to social workers and interdisciplinary team members. Develops, plans and facilitates in-service training programs and works with individual staff to identify learning needs and necessary educational programs and literature to meet their needs in order to develop their professional/clinical knowledge and skills. Constructs orientation schedules and time frames for meeting competencies and VACO licensing and certification standards are established for new social work staff. Serves as a mentor to other social workers, particularly those at the entry level. Program Evaluation: Provides timely and accurate information both verbally and in writing for reports to leadership. Develops and implements program specific guidelines and policies as required. The incumbent keeps staff fully informed at monthly staff meetings of changes as they occur within their program. Explains Veteran's treatment and progress to third parties as appropriate. Manages the quality control and evaluation of the professional services provided. Devises methods of obtaining data and evaluates data to determine the effectiveness of treatment. Coordinates team reviews of medical records and assures that the documentation of the assigned staff reflects the services provided and meets requirements of JCAHO, VHA, Medical Center, and Care Management and Social Work Service documentation policies and standards. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken & written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to SW Licensure Summary by State. Grade Determinations: Supervisory Social Worker, GS-12 Education and Experience: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: Ability to independently organize work, set priorities, and meet multiple deadlines. Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement. Ability to ensure provision of clinical social work services. Ability to provide staff training and development. Skill in interpersonal relationships in dealing with employees, team leaders, and managers. Preferred Experience: Current medical or hospital social worker. The actual grade at which an applicant may be selected for this vacancy is 12. Physical Requirements: The work is sedentary. Typically, the employee sits comfortably while working. However, there may be some light exertion required when walking, standing, bending, climbing stairs, carrying of light items such as papers, books, small objects, or driving an automobile. No special physical demands are required to perform the work. Travel is required within the Eastern Colorado VA Healthcare Catchment area. Incumbent must possess and maintain a current, valid, unrestricted valid state driver's license.
