Fargo Public Schools Additional Information: Show/Hide - Conduct or coordinate, under the direction of the building principal, transition assessments to identify students’ strengths, needs, preferences, and post-placement goals.
- Provide one-on-one and small-group support to help students adjust academically, socially, and emotionally to new settings, including public high schools.
- Guide students in developing executive functioning skills such as organization, time management, study habits, self-advocacy, and goal setting.
- Serve as a consistent, trusted adult advocate for assigned students, fostering connection and resilience throughout the transition process.
- Provide de-escalation and crisis support in collaboration with the leadership team.
- Support classroom teachers with trauma-informed strategies.
- Collaborate, under the direction of the building principal, with DMS, home school districts, residential staff, families, referral sources, and community providers to develop and implement individualized transition plans.
- Track academic performance, communicate with teachers, and assist in providing timely interventions or referrals.
- Participate in discharge planning meetings and provide regular updates on student progress to special education case managers.
- Monitor attendance, behavior, and engagement data; identify early warning signs and intervene proactively.
- Understand and support the implementation of IEPs and 504 Plans; advocate for accommodations and modifications that foster success.
- Maintain accurate documentation and contribute to data collection for reporting and program improvement.
- Connect students to supplemental services such as tutoring, mentoring, post-secondary planning, and behavioral health resources as directed by the education teacher.
- Facilitate warm hand-offs to community-based supports such as therapists, youth mentors, or school counselors
- Complete additional projects as assigned.
- Participate in all mandatory training or training appropriate to the position as required by the District and Dakota Memorial School.
- High school diploma or equivalent required.
- Minimum of three years of experience working in a school setting.
- Experience supporting students with behavioral, emotional, and trauma-related needs.
- Strong interpersonal communication, collaboration, and organizational skills.
- Ability to work across multidisciplinary teams and build effective partnerships with external agencies.
- Post-secondary degree or coursework.
- Background in alternative, residential, or therapeutic education environments.

Position Purpose: The Student Transition Specialist supports successful academic, behavioral, and emotional transitions for students entering, attending, and exiting Dakota Memorial School (DMS). This position serves as a vital liaison among DMS, home school districts, families, referral sources, and community service providers to ensure personalized, coordinated, and compassionate support. Through direct student engagement, collaborative planning, and school leadership direction, the specialist helps students prepare for reintegration into less restrictive learning environments and fosters continuity of care across settings. This position is located at Dakota Boys and Girls Ranch & is contingent upon the availability of grant funding.

This position reports to: Building Principal
Personal contact with students, staff, parents/guardians, faculty, and administrators is an essential part of the position and requires the ability to share information and must be sensitive to people and their needs. The position may handle confidential and private data.
Work involves differing situations and the ability to rapidly access and prioritize competing demands. It is important to identify the problem and resolve it promptly and tactfully?.
The incumbent performs tasks under the direction of building principal and responds to day-to-day needs, answer questions, and responds to emergent issues. Non-emergent issues are discussed with the building principal.
The incumbent takes the initiative to ensure completion of all tasks and will adjust priorities to accommodate competing workload demands. All employees must comply with department and institution procedures and policies, Fargo Public School policies and procedures, as well as local, state and federal laws, regulations, guidelines and business and industry standards.
