Residential Case Manager
$24-$26 Hourly
(Mon-Fri 9:00 a.m. - 5:00 p.m.)
We are looking for a Case Manager to enhance the quality of our client management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle. Case Manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed.
The Residential Case Manager will coordinate client services per treatment plan for the adult clientele. The case manager will be assigned specific clients at time of intake and will be required to program and monitor such things as court appearances, doctor appointments, family therapy sessions, and discharge referrals. The Residential Case Manager will also maintain open communication and/or meet with family, courts, and others as necessary. The Residential Case Manager will meet with members of the Treatment Team.
Essential Duties and Responsibilities:
- Maintain caseload of active clients that require case management in such areas as programming court appearances, doctor appointments, family therapy sessions, and discharge referrals.
- Act as liaison between courts, families, and outside agencies.
- Provide case management services in accordance with the service plan as appropriate to the program.
- Coordinate with the members of the Treatment Team and provide them the appropriate reports prior to meeting.
- Completes documentation and reports as necessary.
- May be required to transport clients in an agency vehicle to meet program needs.
- Provides on-call services as needed.
- Attends staff meetings as necessary and participates as a team member.
- Upholds total confidentiality with respect to clients in all instances.
- Attends training and in-services as required.
- Ensure that each client is registered for food stamps. Follow up on the completion of the process, working jointly with administrative Case Manager.
- Ensure that authorization for Medicaid clients are in on time.
- Monitor the billing data/units for Medicaid.
- Maintain client case management log book in accordance with Program/Contract guidelines.

Education and/or Experience:
- Bachelor’s degree in Psychology, social work, mental health or related field.
- One year experience in counseling or case management with Severely Emotionally Disturbed adult/children
- Possess targeted case manager certification or obtain certification within 90 days of employment.
