Are you an organized, proactive professional with a passion for making a difference? We’re looking for a dedicated Patient Care Coordinator to ensure smooth operations and support compassionate care at our Canon City office.
What You’ll Do:
Coordinate Patient Care: Manage CNA schedules and patient caseloads, ensuring seamless daily visits.
Support the Team: Address clinician needs, cover shifts, and handle administrative tasks with finesse.
Ensure Compliance: Collaborate with Medical Records and Compliance to maintain top-quality care aligned with care plans.
Grow and Adapt: Identify training needs and work with education departments to keep staff at their best.

Requirements
- High School Diploma and at least 1 year of office experience
- Scheduling Skills: 6+ months of experience coordinating teams is a plus!
- Organizational Excellence: Stay on top of schedules, calls, and documentation
- Tech-Savvy: Proficiency in Excel, Word, Outlook, and typing 34+ WPM
- Strong Communication: Connect effectively with staff and patients alike
